| BackgroundIschemic cerebrovascular disease is one of the highest rates of disability and death disease in the worldwide, seriously harm to human health and quality of life, to bring a heavy burden on society and the family. while aging society in China, the incidence rate of ischemic cerebrovascular disease is gradually increasing ,age of onset tends to be younger, as primary and secondary prevention of the ischemic cerebrovascular disease, endovascular treatment technology characteristics in clinical in 1991,with its minimally invasive and he rapid development of neurovascular imaging technology and equipment, The percutaneous transluminal cerebral angioplasty stenting (PTAS) treatment of intracranial and extracranial artery stenosis is beginning the worldwide rapid promotion, has become an important means of the treatment of ischemic cerebrovascular disease.However, with percutaneous transluminal cerebral angioplasty and stenting (PTAS) treatment of intracranial and extracranial artery stenosis widely carried out, the reports of complications have increased gradually, mostly for thrombosis, vascular occlusion,restenosis and high perfusion of ischemic events, serious impact on the prognosis results of patients, so the degree of benefit for endovascular treatment of ischemic cerebrovascular disease is particularly important, by the research of benefit for endovascular treatment of ischemic cerebrovascular disease to improve awareness of endovascular therapy for ischemic cerebrovascular disease ,to reduce postoperative complications and promote vascular interventional technology.In recent years, as multi-mode neurovascular imaging technology to image,the ischemic cerebrovascular disease diagnosis has been significantly improved, the continuous improvement of post-processing software and multi-slice spiral CT (MSCT) scanning technology enhance efficacy of preoperative evaluation and the value of measure for the multi-mode CT in the absence interventional treatment of hemorrhagic cerebrovascular disease, is playing an increasingly important, even decisive role. The project reviewing studies the benefit of endovascular treatment for ischemic cerebrovascular disease from multi-modal imaging in the nerves and blood vessels, expected to find indications that not all have the surgery patients with ischemic cerebral vascular disease stenting had a higher benefit by the study, but rather in the context of a certain lesions in patients with vascular stent implantation have a higher benefit , and thus improve the stent placement in the treatment of indications screen, the enhancement of targeted and effective intervention for the benefit of large ischemic stroke patients.ObjectiveBy researching the efficacy of preoperative evaluation and the value of measure of ischemic neurovascular interventional therapy for cerebrovascular disease to change the traditional awareness of ischemic cerebrovascular disease through a single neurological symptoms and signs, or DSA.MethodsTwo sections of this study was described,for the first part of this group of patients admitted to hospital, the first choice is general CT scan examination to exclude cerebral hemorrhage ,and thenis the CTP CTA examination, the patients met the stenting procedure pointer are for preoperative stenting assessment. The second part is evaluation of the implantation of patients after stent.In the first part of the preoperative evaluation of stent implantation, we head MOSAIC (multimodal stroke assessment using computed tomography) score and ASPECTS(Alberta stroke program early CT score) scores with TTP perfusion parametersIn the second part of the assessment after stent implantation, we head ASPECTS score with TTP perfusion parameters within 7 days after stent implantation in patients。Comprehensing the two part of the research data to calculate the TTP improvement before and after stent implantation in patients, and analyse the preoperative between MOSAIC score and the TTP improvement. ResultsThe 26 patients were divideded into 4 groups with score of 4 point , 5 point, 6 point , 7 point and the improvement degree was increase by 31.7±14.17,38.6±15.73,43.3±10.3,358.6±13.45(%),the pairedt test demonstrated that there were a statistically significant difference (H=10.673 P﹤0.05)Preoperative MOSAIC score was positively correlated with the improvement of ASPECT score for TTP , with a correlation coefficient of 0.579 (P<0.002).ConclusionsMultimodal CT is a sensitive assessment for the evaluation of ischemic cerebrov- ascular disease, especially in the base of Multimodal CT, Patients with a high MOSAIC score may benefit more from stent implantation. |
