Abstract:Objective:To study the expression and significance of TNF-α, MMP-9, to study their correlation with each other and their relationship with the infiltration of eosinophile granulocyte in nasal polyps, for further exploring the mechanism and significance of infiltration process of eosinophile granulocyte. Methods:The experiment group was nasal polyps getting from 30 patients by FESS operation, including 21 men and 9 women, whose average age was 46 year old. The control group was inferior turbinate tissue from 10 patients with simple inferior turbinate hypertrophy or deviation of nasal septum followed with inferior turbinate hypertrophy, including 4 men and 6 women, whose average age is 34 year old. All the patients with asthma or allergic rhinitis should be removed, and all the patients had not taken steroid and antihistamine medicine for near two weeks. All the tissues were fixed in 4% formaldehyde with 1‰DEPC in time and then were embeded and sliced. The expression of TNF-αand MMP-9 mRNA were determined by in situ hybridization technique in tissues. Their expression levels of mRNA were detected by counting positive cells each field of vision and their expression levels in blood vessels were detected by counting positive blood vessels each field of vision. The expression of TNF-αand MMP-9 protein were determined by immunohistochemical technique (SP method) in tissues. Their expression levels of protein were detected by counting positive cells each field of vision and their expression levels in blood vessels were detected by counting positive blood vessels each field of vision. Eosinophile granulocyte was detected in the same tissues by HE staining and its number was counted each field of vision. Their correlations with each other were also analyzed in the tissues of nasal polyps by correlation analysis. Results:The number of eosinophile granulocyte in nasal polyps was obviously larger than that of control group (P<0.05). The number of TNF-a positive cells and TNF-a positive blood vessels were larger in nasal polyps than that in control group(P<0.05).The number of MMP-9 positive cells was larger in nasal polyps than that in control group(P<0.05). But only small amounts of MMP-9 positive blood vessels were detected in the tissues of nasal polyps, and the number of MMP-9 positive blood vessels was not significantly different between the experimental group and the control group(P>0.05). There were positive relationship between the number of TNF-a positive cells and blood vessels and the number of eosinophile granulocyte, but there was only positive relationship between the number of MMP-9 positive cells and eosinophile granulocyte (P<0.05). At the same time there was positive relationship between the number of TNF-a and MMP-9 positive cells(P<0.05). Results detected by in situ hybridization and immunohistochemical technique were coincident. Conclusions:The study indicated that the infiltration of eosinophile granulocyte into both the epithelial and subepithelial layers may play a part in the process of nasal polyps morbility, and eosinophile granulocyte can produce much cytokine and mediator of inflammation which can induce pathological reaction. This mechanism may be an important factor in the process of pathopoiesis. It suggested that TNF-a and MMP-9 may play an important role in the pathological mechanism of nasal polyps. TNF-a can prompt migration of eosinophile granulocyte effectively by direct and mediate pathways. TNF-a may induce the produce of MMP-9 from rhagiocrine cell and inflammation cell, and MMP-9 participate in migration of eosinophile granulocyte by degradation of intercellular substance and basement membrane. In addition, the condition and level of expression examined by in situ hybridization and immunohistochemical technique were almost coincident, which suggested that the produce of TNF-a and MMP-9 protein was regulated at the level of mRNA transcription.
