Background:Children's cough variant asthma (CVA) is a special type of asthma, which is based on the long-term chronic cough as the main clinical feature and is one of the main reasons of chronic cough in children. In recent years, CVA has a rising trend in morbidity due to the effects of genetics, living environment, climate and so on. CVA was often misdiagnosed as upper respiratory tract infections, bronchitis, chronic pharyngitis and so on, because its clinical symptoms are without typical wheezing. Those bring great mental and economic burdens to the patients and their parents. As a result, more and more medical workers at home or abroad pay attention to CVA's prevention and treatment recently, but there is still lack of in-depth study about TCM symptoms of CVA, the traditional diagnosis is too subjective to reflect it's TCM differentiation rules completely and objectively, which is judged by Western medical diagnostic criteria.Clinical research:[purpose] The study quantified the TCM symptoms of children's CVA and came out that clinical TCM differentiation rules was the certain form which determined the function relation among differentiations by the way of mathematics. Based on literature studies and related symptom-signs researches, a reasonable discriminant function was initially established according to the four main syndromes which were the wind-cold invasion of lung, wind-heat invasion of lung, liver-fire invasion of lung and deficiency of lung-Qi; so as to expound the children's CVA symptom-signs'characteristics with quantitative function relation of four examination information, make some preliminary exploration for the quantitative research on children's CVA symptom-signs'characteristics, establish a good foundation for the objective and standardized differentiation of children's CVA, and increase the strength of the argument of the syndrome standardization. [Method] Firstly, the study collected clinical symptom-signs and related materials of children who had been diagnosed as CVA strictly according to the standard in Jiangsu Province Hospital during 2008.12 to 2010.03. Secondly, filled out the "clinical symptom-signs observation table of children of CVA" and differentiated their correct TCM syndromes; then summarized their common symptom-signs, divided the quantization scale by the symptom-signs'severity level, work out the score criterion and made them scored. At last, a Bayes discriminant function was established for each syndrome by using Bayes discriminant analysis gradually. [Results] The wind-cold invasion of lung:Y=-20.314+0.998 sweating+ 6.068 nasal discharge+0.101 pharynx+0.565 tonsil+4.623 reddened tongue+1.820 pale tongue+5.041 thin and slightly yellow coating+0.166 thready pulse (slightly red)+3.408 taut and rapid pulse (purple and knotted)+31.906 floating and tense pulse (floating and red)+4.699 floating and rapid pulse (floating and purple); the wind-heat invasion of lung:Y=-21.321+4.888 sweating+3.201 nasal discharge+3.581 pharynx+4.789 tonsil+9.856 reddened tongue+6.760 pale tongue+4.256 thin and slightly yellow coating-1.681 thready pulse (slightly red)+11.324 taut and rapid pulse (purple and knotted)+11.587 floating and tense pulse (floating and red)+6.855 floating and rapid pulse (floating and purple); the liver-fire invasion of lung: Y=-26.895+5.726 sweating+2.865 nasal discharge-1.975 pharynx+2.814 tonsil+17.586 reddened tongue+8.465 pale tongue+11.892 thin and slightly yellow coating-4.463 thready pulse (slightly red)+19.705 taut and rapid pulse (purple and knotted)+10.349 floating and tense pulse (floating and red)-1.970 floating and rapid pulse (floating and purple); the deficiency of lung-Qi:Y=-24.132+10.907 sweating+0.680 nasal discharge+2.059 pharynx-3.400 tonsil +10.182 reddened tongue+15.976 pale tongue-6.037 thin and slightly yellow coating+15.317 thready pulse (slightly red)-2.412 taut and rapid pulse (purple and knotted)-0.769 floating and tense pulse (floating and red)-2.218 floating and rapid pulse (floating and purple). Later that discriminant function had been used backly to have it tested, the correct percentages of that discriminant in various syndromes were described below, the correct rates of the wind-cold invasion of lung was 95.0%, wind-heat invasion of lung was 96.9%, liver-fire invasion of lung was 85.7% and deficiency of lung-Qi was 100.0%. The discriminanting results of various syndromes and the differentiation of traditional experiences were similar. It showed that the discriminant function had certain value in the differentiation of children's CVA.Conclusion:Bayes discriminant functions of various syndromes were established by quantifying children's CVA common TCM symptom-signs and summarizing the regularity of the clinical TCM differentiation and treatment, these functions had some value in making the differentiation and treatment of the children's CVA objective and standard.
