Objective Analysis of factors related to fetal birth weight; To investigate the clinical value of B ultrasound (B-US) in measuring the thickness of soft tissue of humerus of fetus in forecasting newborn birth weight. To seek a more accurate equation for estimating the macrosomia weight.Method The biparietal diameter (BPD), head circumference (HC), abdom inal circumference (AC),femur length (FL), humeral soft tissue thickness (HSTT),fumeral soft tissue thickness (FSTT), fetal kidney volume (KV)(KV=0.439 X bilateral kidney length X kidney width X kidney thickness) of 307 singleton fetuses were measured by two-dimensional ultrasound. All infants were delivererd whinin 1 week afer the examinations. To study the correlation between all parameters and the birth weight;Using humeral soft tissue thickness as the independent variable and birth weight as the dependent variable drawn scatter plot, To derive predicting fetal weight of a single parameter and multi-parameter formula by The multiple regression. Comparing the two formulas, and compared with those of formulas commonly used in clinical practices respectively; According to the birth weight two groups were divided:macrosomic and nonmacrosomic groups, and the characterstics of two groups were compared; the relationship between humeral soft tissue thickness (HSTT) and macrosomic birth weight was analyzed. Find the cut-off value, specific and sensitivity of predict macrosomic by measure fumeral soft tissue thickness use ROC curve.Rusults Various parameters (BPD, HC, AC, FL, HSTT, FSTT and KV) and fetal weight were linearly correlated, which humeral soft tissue thickness and abdominal circumference are highly correlated (respectivelyγ=0.821 andγ=0.865), single-parameter formula is derived BW=1083.294+2099.853 X HSTT; multi-parameter formula BW=-2900.247+103.927 X AC+787.947 X HSTT +282.036×FSTT+96.712×FL; HSTT (γ=0.833) correlation was higher than AC (γ=0.791) on prediction macrosomic birth weight, Forecast for macrosomic sensitivity 90.16%, specificity was 90.01%. when HSTT≥1.315cm obtained by ROC curve.Conclusions1 A humeral soft tissue thickness can be an indicator for predicting fetal weight.2 Combined measure abdominal circumference (AC)and such as humeral soft tissue thickness (HSTT) parameters is high accuracy in predict fetal borth weight.3 Humeral soft tissue thickness≥1.315cm may initially considered for large children, so as to provide more clinical simple, accurate way to predict macrosomic.
