Objective:To discuss the relationship between fetal macrosomia andultrasonic measurement of fetal abdominal circumference,To find the bestcut-off value of fetal abdominal circumference for predicting macrosomia by thereceiver operating characteristic curve,and analysis fetal macrosomia influenceon the mode of delivery and pregnancy outcomes.Method: The clinical information of545cases of fetal macrosomia(≥4000g)and the same period of clinical information of600cases of normalweight newborn(2500g-3999g)were analyzed retrospectively in LiuzhouPeople’s Hospital of Obstetrics during January1,2011to December31,2013.Using pearson correlation analysis to explicit the relationship between ultrasoniccommon measurement (fetal biparietal diameter、head circumference、abdominalcircumference、femur length)and macrosomia birth weight. By the receiver operating characteristic curve of the fetal abdominal circumference to determinethe diagnostic value and finding the best cut-off value.Resluts:1.Correlation analysis shows positive correlation among theultrasonic measurement(fetal biparietal diameter, head circumference,abdominal circumference, femur length)and macrosomia birth weight;2.The areaunder the receiver operating characteristic curves of the ultrasonic measurementof fetal abdominal circumference predicted fetal macrosomia is0.955,Accordingto Youden index to determine the best cut-off value is355mm;3.The rate ofcesarean section、cephalopelvic disproportion、abnormal labor、persistent occiputposterior position or persistent occiput transverse position、maternal birthtrauma、the rate of postpartum hemorrhage、birth trauma、fetal distress、neonataldistress in fetal macrosomia group were higher than the normal child group, thedifference was statistically significant (P<0.05).Conclusion:1.Ultrasonic measurement of fetal abdominal circumference forpredicting macrosomia is a better indicator and the best cut-off value is355mm.2.Fetal macrosomia may bring lots of adverse pregnancy outcomes, Predict themacrosomia accurately and choose the right mode of delivery and deliveryopportunity is very important to reduce the incidence of maternal and infantcomplications.
