| Objective To investigate the effective of Shenfu Injection (SFI) on erythrocyte immune function of Patients undergoing valve replacement. Methods Twenty adult patients scheduled for Undergoing Valve Replacement were randomly divided into two groups:SFI group (group F, n=10) and the control group(group C, n=10). Patients were established Intravenous access then undergone arteriovenous invasive puncture and BP, HR, CVP, ECG,SpO2 were monitored on two groups of patients in the operating room, in which endotracheal intubation with inhalational combined intravenous anesthesia after rapid induction. During cardio-pulmonary bypass(CPB) was established after the median sternotomy. SFI 1.5ml.kg-1 was dripped intravenously after induction of anesthesia in group F. While it was treated with only physiological salt solution in group C, blood samples(4ml) were collected from artery before induction of anesthesia (T1), CPB 30 minuters(T2), end of CPB immediately(T3) and postoperative 24 hours(T4) respectively in all groups, the blood samples were centrifuged by 2000 r/min for 3min at room temperature, plasma from the upper level was conserved in-70℃freezer and erythrocyte was conserved in 4℃freezer. Blood plasm interleukin-6(IL-6)was detected with ELISA, blood plasm malondialdehyde(MDA)and free hemoglobin(FHB)was detected with ultraviolet spectrophotometer, the rosette rate of RBC-C3b receptor (RBC-C3bRR) and RBC-immune complex (RBC-ICR) was detected with way of yeast rose chaplet at today. Results There were no significant differences in the levels of RBC-C3bRR, RBC-ICR, IL-6, MDA, FHB before induction of anesthesia in both groups(P>0.05). Compared between group C and group F:RBC-C3bRR was lower and RBC-ICR, IL-6, MDA, FHB were higher in the group C than those in the group F at CPB 30 minutes and after it every time points and there was significant differences between the group C and the group F(P<0.05). Compared within group:RBC-C3bRR was lower and RBC-ICR, IL-6, MDA, FHB were higher than that before induction of anesthesia in both groups at CPB 30 minutes, end of CPB immediately, postoperative 24 hours; After CPB beginning and along with CPB temporal extend, RBC-C3bRR gradually decrease and RBC-ICR, IL-6, MDA, FHB gradually increase in both group. RBC-C3bRR, RBC-ICR, IL-6, MDA and FHB were well than those in CPB, but they irreparability the same as before induction of anesthesia. Conclusion erythrocyte immune function suffered secondary lesion in CPB. SFI may degrade concentration of blood plasm IL-6 and MDA and have better protection of erythrocyte immune function undergoing valve replacement... |
