To Research The Structure And Esophageal Barrier Function Of Excluded Esophagus After Esophageal Diversion Surgery In Acquired Tracheoesophageal Fistula

Objective:To explore the changes of structure and esophageal barrier function of excluded esophagus after esophageal diversion surgery in acquired tracheoesophageal fistula.Methods:15 canis with the fistula was excluded (A group),15 canis with the esophagus was single excluded but not fistula(B group),5 canis with normal esophagus(C group) to be observed the esophageal sample, H-E, transmission electron microscope(TEM), the scanning of electron microscope(SEM) and Lanthanum trace labeling,the interstitial space dilation(ISD) was measured and the number of desmosome in the intercellular space was calculated by using a digital scanner form photomicrographs of the specimens.Results: Changes of structure and esophageal barrier function of excluded esophagus in A group was identical with B group. Compared with C group,A and B group showed following changes:①esophageal structure was changed; glandular organ in undermucous was reduced and deleted even,②the score of the interstitial space was increased; the number of desmosome was decreased, and the esophageal barrier function was attenuated, but the basel lamina was not changed; lanthanum did not break throught it,③the interstitial space was filled with Lanthanum and its shape is lineer,④the mean number of desmosome among A group,B group,C group were 0.214±0.034entries/μ㎡, 0.221±0.045 entries/μ㎡, 0.322±0.033 entries/μ㎡ respectively,there were no significant difference between A group and B group(P﹥0.05).The mean number of desmosome between A group and B group,much lower than the C group(P﹤0.01).The mean score of the interstitial space among A group,B group,C group were 2.11±0.56μm, 2.04±0.77μm, 1.84±0.49μm respectively,they was no significant difference between A group and B group(P﹥0.05).The mean score the interstitial space between A group and B group,much higher than C group(P﹤0.05).Conlusion:For the refractory acquired TEF,esophageal exclusion surgery was convenient, easy, and effective. So the method deserves to be generalized.
