Objective:The aim of this study was to investigate whether the polymorphisms of the tissue factor pathway inhibitor (TFPI-1) gene promoter region contribute to acute coronary syndrome or modify plasma TFPI-1 levels.Methods:Patients(59) who had a recently history of ACS, patients(36) who had a CAD history, and 38 patients with negative CAG results as control group, ages around 40-70, were recruited. DNA was extracted from EDTA whole blood taken from vein. A 1068 bp region of the TFPI promoter was amplified by PCR and its sequence was analyzed. Plasma TFPI level of 60 patients, ages around 50-59, who had no history of thrombosis or heparin using in 3 months was measured. Investigate the possible relationship between variations of TFPI gene promoter region and ACS or the plasma level of TFPI. All analysis was performed using the SPSS 17.0.Results:1. We found no new polymorphisms in the promoter region of TFPI-1 gene other than-287T-C and-399C-T.2. Comparison of-287T-C and-399C-T polymorphism frequencies in ACS patients, SA patients and control subjects showed no evidence of a link with the risk of coronary heart disease.3. We found that the polymorphism of the-287T-C might have a possible relationship with the subtype of ACS. STEMI Patients had a lower percentage of C/C genotype. Whereas UA patients had a higher percentage of C/C genotype, and a lower percentage of T/T genotype. However, larger study will be needed.4. Our results showed the association of the-287C allele with a significant increase in total TFPI antigen levels. While the polymorphism of-399C-T was not associated with a variation in TFPI levels. Conclusion:1. There are two SNP sites in the promoter region of TFPI-1 gene which are-287T-C and-399C-T.2. Polymorphism of promoter region of TFPI-1 gene has no relationship with ACS.3. The polymorphism of the-287T-C might have a possible relationship with the subtype of ACS. The C mutation may have protective effect on preventing ST-elevation myocardial infarction.4. The-287C allele with a significant increase in total TFPI antigen levels. While the polymorphism of-399C-T was not associated with a variation in TFPI levels.
