ObjectiveDetect the expression of glucocorticoid receptor (GR) in Peripheral blood mononuclear cells (PBMC) from patients with pemphigus and the effect of 5-azacytidine and 4- phenyl butyric acid (PBA) on glucocorticoid receptorα(GRα) andβ(GRβ) expression in PBMC of healthy people.Methods1.12 cases of pemphigus patients with glucocorticoid-sensitive,8 cases with glucocorticoid resistance and 10 cases of normal individuals were selected, using semi-quantitative reverse transcription-polymerase chain reaction(RT-PCR) to detect expression GRa and GRβmRNA in PBMC.2. PBMC separated from 4 cases of healthy people were incultured in vitro and treated with different concentrations of 5-aza or PBA. Realtime Polymerase Chain Reaction (Realtime-PCR) was used to detect the expressions of GRa and GRβmRNA.Results1.The expression of GRa mRNA in Pemphigus patients was significantly lower than that in normal control group, while there has no statistical difference between the GC-sensitive and resistant groups; the expression levelof GRP mRNA in GC-resistant group was significantly higher than that both in GC-sensitive group and the normal control group.2.After treatment of PBMC with 5-aza at 3μM concentration, the expression level of GRa mRNA increased compared with the untreated group,.,while the expression of GRβmRNA had no significant change 1μM PBA induced the increase of GRP mRNA expression, while 3μM PBA induced the increase of GRa mRNA expression. After the mixed culture of the two drugs, the expression of GRa significantly increased and the expression of GRβhad no significant change, compared with the treated groups by only one drug.Conclusion 1.The high expression of GRβmay be related to glucocorticoid resistance in Pemphigus patients.2.GRa and GRβtranscription and translation may be reglulated by the level of the overall gene methylation and acetylation.
