Objective: To investigate the advantages and disadvantages of different routes of subclavian vein puncture through infraclavicular approach for the central venous cannulation.Methods: 106 candidates were divided into the medial group (MG) of 63 cases whose puncture site is at the junction of the medial and middle thirds of the clavicle and then 1.0 cm below the clavicle, and the direction is toward the posterosuperior corner of the sternoclavicular joint, and the lateral group (LG) of 43 cases whose puncture site is 1.0-2.0 cms below the clavicle of its midline and then 0.5-1.0 cm outside the midline of the clavicle, and the direction is toward the superior rim of the angle between the sternal head of sternocleidomastoid muscle and the clavicle. The successful rate, operative time and complication rate were compared between the two groups.Results: The first puncture successful rate in LG is significantly higher than that of MG (76.7% VS 57.1%,P <0.05). The overall puncture successful rate was 97.7% in LG and 93.7% in MG (P> 0.05). The average operative time was 23.7±14.6 minutes in LG and 28.6±17.3 minutes in MG (P> 0.05). The complication rate was 2.3% in LG and 4.8% in MG (P> 0.05).Conclusion: The two routes of of subclavian vian puncture through infraclavicular approach for the central venous cannulation are both feasible, but LG is better than MG in the first puncture successful rate.
