Nonalcoholic fatty liver (NAFLD) is a chronic liver disease and has no history of excessive alcohol consumption whose clinicopathological features are liver steatosis, ballooning degeneration, mild diffuse lobular inflammation and hepatocyte necrosis. Nonalcoholic steatohepatitis (NASH) is an important intermediate link when NAFLD converts to liver fibrosis and cirrhosis, which mainly relates to oxidative stress and lipid peroxidation damage. Now there are numerous clinical drugs for the treatment of fatty liver, and Chinese herbal compound occupies an increasingly important place.Baohewan, from Danxi Xinfa, has the function of improving digestion, dispelling stagnation and phlegm, and broking stasis. Clinical observations show that Baohewan has a certain effect in the treatment of hyperlipidemia, indigestion and other digestive system diseases. Huzhang is the root and stem of the Polygonum, and its active ingredients, resveratrol and resveratrol glucoside, have the pharmacological effects of lowering blood fat and sugar, antioxidant, protecting gallbladder and liver, improving blood circulation and so on. Currently Huzhang is clinically used for the treatment of liver diseases with other traditional Chinese medicine.In this study, we constructed the nonalcoholic steatohepatitis model in rats induced by fat emulsion, and then researched the treatment effects and partial mechanism of Baohewan and Baohewan added with Huzhang in NASH. The main contents summarized as follows:1. Protective effects of Baohewan and Baohewan added with Huzhang in SD rats with NASHSD rats were given high fat emulsion and then induced NASH model. After 3 weeks, part of them were given different doses of Baohewan, Baohewan added with Huzhang or tiopronin at the same time, and then 9 weeks after constructing the model, compared with model group, Baohewan and Baohewan added with Huzhang could decrease serum ALT, AST, LDL, TC, TG,levels and increase serum HDL level significantly. Histopathology showed Baohewan and Baohewan added with Huzhang could significantly reduce the degree of liver fatty degeneration and inflammation. These results suggested that Baohewan and Baohewan added with Huzhang had obvious therapeutic effects in rats with NASH induced by high fat emulsion.2. The effects of Baohewan and Baohewan added with Huzhang on oxidative metabolism in SD rats with NASHCompared with model group, Baohewan and Baohewan added with Huzhang could significantly reduce the serum MDA level, lower insulin resistance index, increase SOD and the activity level of adiponectin, meanwhile inhibit the expression of CYP2E1 in liver. Electron microscopy showed the swelling of mitochondria and lipid droplets from treated group were reduced significantly. These results suggested that the therapeutic effects of Baohewan and Baohewan added with Huzhang in the SD rats with NASH maybe relate to the regulation of lipid metabolism, the improvement of insulin resistance, the inhibition of CYP2E1 expression and the reduction of fat deposition.3. The effects of Baohewan and Baohewan added with Huzhang on inflammatory response in SD rats with NASH.Compared with model group, Baohewan and Baohewan added with Huzhang could significantly reduce the content of TNF-αin serum, and the high dose group could significantly inhibit the expression of NFκBp65 in the liver nuclei of rats with NASH induced by high fat emulsion. These results suggested that the therapeutic effects of Baohewan and Baohewan added with Huzhang in the SD rats with NASH maybe associate with the inhibition of cytokines expression and reduction of inflammatory response.To sum up, Baohewan and Baohewan added with Huzhang had obvious therapeutic effects in rats with NASH induced by high fat emulsion and the mechanism may relate to the regulation of lipid metabolism, the improvement of insulin resistance, the reduction of CYP2E1 expression in liver mitochondria, the protection of mitochondrial structure and function, the inhibition of inflammatory cytokines expression and so on.
