Lung cancer as a serious disease is harm to human health, "the incidence first" and "mortality first" has been increasingly recognized by scholars at home and abroad. Growth and metastasis of lung cancer are the result of multiple factors, multi-link together. Above all, angiogenesis of lung cancer is the anatomical and physiological basis for invasion and metastasis of lung cancer, to suppress angiogenesis and cut off lifeline of the growth and metastasis has become the new strategy for clinical prevention and treatment of lung cancer. And it's specificity suppressor or receptor antagonist may be useful to suppress certain tumors cells proliferation and invasion, thus it may be an important tumor cell survival factor underlying human carcinogenesis.Recently studies have reported that adrenomedullin and its receptor are found in a variety of tumor lines and tissues, they may play a critical role in cancer cell division, tumor neovascularization, circumvention of programmed cell death, and may help cancer cells to circumvent immune surveillance.Studies have shown that vascular endothelial growth factor has been well-recognized to be the strongest, highest-specific regulation of angiogenic factor among numerous angiogenesis factors. The targets treatment for VEGF and VEGF receptor (VEGFR) are the most ideal targets of anti-angiogenesis However, the expression of ADM and its correlation with VEGF and other angiogenesis factors in plasma and tissues of patients with lung cancer has no system research both at home and abroad.Objective:To investigate the expression of ADM and VEGF in plasma and tissue from surgical resection of patients with lung cancer, and to detect the relationship among the expression of ADM and VEGF and their correlation with TNM staging, pathologic types, lymph node metastasis and other biocharacteristics of the mutual relations. To investigate the role of ADM in lung cancer angiogenesis and its potential clinical applications for cancer therapy.Method:1. The plasma levels of ADM and VEGF in 66 lung cancer,31 benign lesions, and 30 healthy control group were measured by radioimmunoassay (RIA), and to detect their relationship with pathologic types, TNM staging and lymphaden metastasis.2. The positive expression rates of ADM and VEGF in 50 lung cancer tissues,27 adjacent-tumor tissues and 15 inflammatory pseudotumour were measured by immunohistochemical (SABC) assay, and to detect the relationship among the expression of ADM, VEGF, MVD, and their correlation with TNM staging, lymph node metastasis, pathologic types, degree of histodifferentiation.Results:1. The expression of ADM and VEGF in lung cancer plasma(1). ADM and VEGF plasma levels in lung cancer group were significantly higher than those of benign lung disease group and healthy group (p<0.05), ADM plasma levels in benign lung disease group were significantly higher than the healthy group (p<0.05), while plasma levels of VEGF in benign lung disease group and healthy group have no significant difference (p> 0.05).(2). ADM and VEGF plasma levels of TNMâ…£were significantly higher than in staging III andâ…¡(p<0.05), while there was no significantly difference between staging III and II (p> 0.05).There were positive relationship among ADM, VEGF and TNM staging (r=0.553, r=0.420, p<0.05).(3). ADM and VEGF plasma levels of distant metastasis in lung cancer group were significantly higher than the lymph node metastasis group (p<0.05), lymph node metastasis group were significantly higher than those without lymph node metastasis group (p<0.05). There were a positive correlation among lymph node metastasis, ADM and VEGF (r=0.441, r=0.433, p<0.05).(4). There was a significant positive correlation between ADM and VEGF plasma levels in the lung cancer group(r=0.449, p<0.01).(5). ADM and VEGF plasma levels of lung cancer have no statistically significant in the pathological type, age, gender difference (p>0.05).2. The expression of ADM, VEGF and the counts of MVD in lung cancer tissue(1). The expression of ADM, VEGF and the counts of microvessel densityn (MVD) in lung cancer tissue were significantly higher than those in adjacent tumor tissues and inflammatory pseudotumor(p<0.05), while the adjacent tumor tissue and inflammatory pseudotumor have no significantly difference(p>0.05).(2). The expression of ADM, VEGF and the counts of microvessel densityn (MVD) in TNMâ…¢andâ…¡have significantly higher than in stagingâ… (p<0.05), while there were no significantly difference between stagingâ…¢andâ…¡period (p>0.05). There were positive relationship among ADM, VEGF, MVD and TNM staging(r=0.437, r=0.642, r=0.655, p<0.05).(3). The expression of ADM, VEGF and the counts of MVD in lymph node metastasis have significantly higher than those without lymph node metastasis (p<0.01). There were positive relationship among ADM, VEGF, MVD and lymph node metastasis(r=0.546, r=0.794, r= 0.560,p<0.05).(4). Correlation analysis to demonstrate that ADM and VEGF, VEGF and MVD, ADM and MVD have significantly correlation, the coefficient correlation are respectively 0.403,0.642,0.622 (p<0.01).(5). The expression of ADM, VEGF and the counts of MVD have no significantly statistically difference in the pathological type, degree of histodifferentiation, age, gender (p>0.05).Conclusions:1. Our study shows that the expression of ADM and VEGF are significantly higher, the trends of ADM and VEGF content in plasma and tissue are basically the same in lung cancer. ADM and VEGF were positively correlated, and correlated with TNM staging, lymph node metastasis, but not with pathological type, histological grade, gender, age and other factors.Tip:ADM and VEGF may be synthesized and secreted by lung cancer cells, they may both as circulating hormones and autocrine/paracrine hormones participate in the development of lung cancer, and plays a synergistic effect. They may be used as indicators for cancer and benign disease's diagnosis and differential diagnosis, clinical TNM staging.2. ADM was significantly higher in the lung cancer tissues than adjacent tissues, inflammatory pseudotumor tissues, and has positively correlated with the expression of VEGF and the counts of microvessel density in lung cancer.Tip:ADM may serve as a new angiogenesis regulatory factors involved in angiogenesis of lung cancer, to interfere with ADM and its receptor pathway is expected to be a potential target for anti-angiogenesis therapy of lung cancer.
