ObjectivesRecurrence is the mainly factor respondent for fatalities of lingual carcinoma patients.The recurrence rate is approximately 30% and associated with several factors especially chewing betel quid in Hunan Province.Recurrence of lingual carcinoma usually involve multiple important anatomic structures, thus making it difficult to entirely resect and reconstruct. Regarding this, we firstly made an epidemiology study to investigate the risk factors of recurrence of lingual carcinoma, and then discussed the invasion extention, with the views of providing guidance for prevention clinical treatments.MethodsAn case-control study was performed between 35 patients with recurrence of lingual carcinoma and 36 patients without recurrence after surgical treatments.The numeral relationship between recurrence of lingual carcinoma and 18 interrelated factors was studied by using the multivariate non-conditional logistic regression analysis.Secondly, a retrospective study was performed on recurrence group mainly focusing on the involved structures,extent of resection and homochronous reconstruction of salvage surgical treatments for Recurrent Lingual Carcinoma.Results1.Among 35 recurrent patients,32 cases had relapsed in 12 months, 2 between 12 and 24 months, and only 1 case out of 2 years.2.7 interrelated factors below were imported to the final logistic regression equation:duration of cigarette smoking, duration of betel quid chewing, the lingual carcinoma accompanied with oral submucous fibrosis(OSF)or oral leukoplakia(OLK) or oral lichen planus(OLP), involvement of tongue base, tongue belly, growth pattern, thickness of tumor.3.In 35 recurrent patients, the local recurrence, cervical recurrence and simultaneous both recurrence occurred in 16 patients(45%),16 patients(45%) and 3 patients(10%), respectively. The local recurrence mainly involved body of tongue,tongue base, lateral oropharyngeal wall, floor of mouth. The cervial recurrece usually involved I and II region.In one case the recurrence tumor metastased to the parotid.Conclusions1.Lingual carcinoma has a trend to relapse in 12 months postoperatively.2.The risk factors of recurrent lingual carcinoma included duration of cigarette smoking, duration of betel quid chewing, the lingual carcinoma accompanied with oral submucous fibrosis(OSF) or oral leukoplakia(OLK) or oral lichen planus(OLP), involvement of tongue base, tongue belly, floor of mouth, growth pattern, thickness of tumor.3.The local recurrence mainly involved body of tongue, tongue base, lateral oropharyngeal wall, floor of mouth. The cervial recurrece usually involved I and II region. The recurrent tumor was rather rare to metastase to the parotid.
