Clinical Analysis On Refractory Malignant Ovarian Germ Cell Tumors

Objective: To investigate the clinical features ,associated factors of the recurrence, and treatment of refractory malignant ovarian germ cell tumors (MOGCT) ,to find available ways to prognosis assessment and more effective treatments.Methods:A clinical cases analysis was carried on 12 patients with recurrent (refractory) malignant ovarian germ cell tumors and 24 recurrence-free MOGCT patients ,who were admitted at the same time period.The differences between the two groups in age ,FIGO stages, surgery, and chemotherapy were compared.The fetures of the recurrent cases were studied.Possible refractory elements , indications and results of different salvage treatment were investigated.Results: There were significant differences between the recurrent group and no-recurrent group in absence of chemotherapy with BEP/BVP, FIGO stages (p<0.05) .No important differences were found in age at onset and reproductive function conservative surgery (p>0.05). Relapse within a short chemotherapy interval(less than 4 weeks) and liver metastasis were negative factors for prognosis of refractory cases,and second cytoreductive surgery was meaningful for pelvic metastasis cases. Chemotherapy was a main salvage therapy for refractory malignant ovarian germ cell tumors.Conclusion:The associated factors of the recurrence of malignant ovarian germ cell tumor may include: advanced stages, absense of combination chemotherapy with BEP/BVP, non- dysgerminoma. Relapse within a short chemotherapy interval and liver metastasis were negative factors for prognosis of refractory cases, and second cytoreductive surgery was meaningful for pelvic metastasis cases. Chemotherapy was a main salvage therapy for refractory malignant ovarian germ cell tumors,to find available salvage chemotherapy was important.
