Clinical Retrospective Analysis Of 34 Malignant Ovarian Germ Cell Tumors

Objective:Malignant germinal cell tumor of ovary predominantly occurs in female young adults.The adjuvant chemotherapy of malignant ovarian germ cell tumors have changed significantly in recent years,which exerts important action in elevating survival rate and the quality of life.In our study,we analysized the clinical information about the 34 patients with malignant ovarian germ cell tumors,and discussed the treatment conditions and the effect of adjuvant chemotherapy on survival and fertility.Methods:Patients' medical records,pathologic type and follow-up information of 34 cases of malignant germ cell tumors of ovary from 1999-2007 in Qilu Hospital of Shandong University were collected.We analysis retrospectively the treatment conditions about the 18 patients,and Kaplan-Meier method were used to analyze the survival rate,and log-rank method were used to analyze the difference of survival rate,P＜0.05 has statistical significance.Results:1,The age of onset was 8-45 years,the mean age was 22.3 years,and the median age was 23 years,presented with symptoms of pelvic mass(23.5%),abdominal pain(55.9%),some patients(41.1 %)were asymptomatic.2,In 34 cases,3 cases have been lost,7 cases have been dead,the total survival rate is 77.4%.Including 19 cases of stageⅠ,2 of stageⅡ,9 of stageⅢand 1 of stageⅣ.The survival of stageⅠ-Ⅱ95.5%hile it was 33.3%in stageⅢandⅣ(P=0.001).3,Patients with adjuvant chemotherapy more than 6 had a survival advantage over those less than 6(86.4%vs 55.6%,P=0.023). 4,Patients with tumor less than 15cm in diameter had not a survival advantage over those more than 15cm(86.7%vs 68.8%,P=0.164).5,Prognosis has no relationship with onset age,part of tumor growth,patho-type and surgery range(P=0.282,0.501,0.970and 0.112 respectively).6,In the 28 cases of preserved fertility,3 cases have been lost,4 cases have been dead,4 cases have not sexual maturity,17 cases have recovered menstrual after operation,9 cases have married and request procreation now,4 cases had been pregnant,3 cases have growth,and their children' s body are health.Conclusion:Pelvic mass and abdominal pain are the most common complaints.We think that the treatment of preserved fertility or endocrine is effective method to treat with malignant ovarian germ cell tumors for young women.Clinicopathologic stage is the important prognostic factor for patients with malignant ovarian germ cell tumors.Post-operation chemotherapy exerts important action for elevating survival rate and the quality of life to process chemotherapy systematically,fully and efficiently.
