Evaluation Of PPHTN By Mlti-slice Spiral CT

Background:PPHTN is sever pulmonary complication of liver cirrhosis. OLT is effective therapeutic meance to advanced liver diseases, but PPHTN is primary cause of high death rate about amb-operation period of OLT. In time and accurate diagnosis of PPHTN is critical to a lower death rate. Imageology is significant means to clinical diagnosis. Mlti-slice spiral ct with high speed of scan and high space resolution, and reconstruction randomly, it has been important meas of diagnosis and pre-operation evaluation to PPHTN.Purpose:To explore the cause of pulmonary hypertension after liver cirrhosis and the feasibility of 16 slices spiral CT in evaluating PPHTN by the diameter mearement of MPA and ascending aorta on the same CT slice and the rPA of them, then distinguished the clinical stage by above two values.Materials and methods:Collect CT image of chest(enhenced or not), liver cirrhosis107(male 78, female 29), normal contrast 166(male 98, female 68), measure the diameters of main pulmonary artery and ascending aorta on the same CT slice, then calculate rPA; The ANOVA test was applied to assess the difference of these diameters among control group,compensation and decompensation group;LSD test was used to the comparision of mean values between every two groups.Results:The MPA diameter of control and compensation group is 25.23±2.99mm, rPA is 0.79±0.10;The MPA diameter of decompensation group is 29.55±3.70mm, rPA is 0.92±0.12. The differences among those groups have statistical significance.Conclusion:PAH after liver cirrhosis have something to do with liver malfunction and portal hypertension all. But portal hypretension(PH) is the major factor. The diameter of MPA may be the index of diagnositic for PPHTN and used to classified compensation and decompensation stage. The clinical practicability of rPA isn't as well as the diameter of MPA. When liver cirrhosis patients'diameter of MPA is larger than 29mm, PAH should be considered.
