objective: to evaluate the efficacy of surface diaphragm EMG in the diagnosis of sleep apnea syndrome.Methods: 10 patients with suspected sleep apnea were enrolled and a conventional full night polysomnography were performed, with the recording of surface and esophageal diaphragm EMG and esophageal pressure simultaneously.Result: 1.during an obstructive apnea event, good correlation were found in surface EMGdi to esophageal EMGdi and surface EMGdi to Peso, with coefficient value of 0.742±0.082 and 0.662±0.089(P<0.05) respectively. Of all sleep apnea events, 269 were marked as obstructive by RIP, while 85 events were judged by Pes and only 71 events were confirmed by surface and esophageal EMGdi. Events judged as obstructive by surface and esophageal EMGdi and Pes are far less than by RIP methods.Conclusion: 1, surface EMGdi correlate well with esophageal EMGdi, and is a reliable method in differentiating obstructive from central apnea. 2, obstructive events may be misclassified as central apnea by RIP, therefore conventional PSG may overestimate central apnea events in certain patients. 3, surface EMGdi and esopeageal EMGdi are more reliable in diagnosing obstructive events than esophageal pressure measurement.
