Background and objectiveGastric Cardia Carcinoma(GCC)is a high incidence of malignant tumors in Chaoshan, theincidence of GCC was 34.81/10,000 , second only to esophageal cancer (EC).Helicobacter pyloriinfection was always found in Gastric Cardia Carcinoma(GCC), it planted in epithelial cells byflagella or taking CD74 as a receptor and injected CagA into cells to activate kinds of signal pathwaysinducing proliferation, differentiation and apoptosis. Extracellular signal-regulated kinase(ERK) is the member of mitogen-activated protein kinase super family(MAPK) and is one of themost important and classical pathways which was closely related to differentiation, tumorigensis,malignant degree,et al. However we still know little about the distribution and function of H.pylori in GCC. Our laboratory has detected distribution of H. pylori in Gastric Cardia Cancer inChaoShan by reform ed Giemsa stain and immunohistochemistry stain. On this basis, this experimentcombined immunohistochemistry of p-ERK protein and SIMPLE technology to observeand analyse the expression of p-ERK in normal mucosa, adjacent nontumorous mucosa and GCCtissue as well as the relationship between distribution of H. pylori and CD74 expression, to investigatethe correlation of distribution of H. pylori and p-ERK expression, and discuss the transmissionof intracellular signaling pathways after the H. pylori infection which may provide certainevidence for further study of the progression of Gastric Cardia Carcinoma.Materials and methodsObject:116 specimens were obtained from patients who underwent biopsy for gastric cardiacarcinoma at Tumor Hospital of Shantou University Medical College from 2007 to 2010(eachspecimen include carcinoma, adjacent and distant cutting margin ), including male 103 andfemale 13, aged 41~81 years, mean age 62.6 years. HE stain:To observe the morphological characteristics of specimens.Immunohistochemistry:To detect the expression of p-ERK protein and CD74 protein.SIMPLE stain:To observe the location relationship of H pylori,p-ERK and CD74 in GCC.Giemsa stain:To observe the colonial density and distribution of H pylori.Image analysis:Images were photographed by Leica IM50 Image acquisition system andlabeling index of p-ERK and CD74 average light density was measured through Pro Plus 6.0Image analysis software.Statistic analysis:We analysed statistical results by variance analysis, Spearman correlationanalysis of SPSS 15.0 software.Results1. There's almost negative expression of p-ERK in normal cardia mucosa.It showed a significantdifference among normal mucosa, adjacent nontumorous mucosa and GCC(F=144.966, p<0.01).2. There's a significant difference of p-ERK expression between H. pylori positive objects and H.pylori negative ones(F=17.159,p<0.01). H. pylori infection(-) ones indicated a significantdifference when it was compared to other infection levels respectively(p<0.01). And there's asignificant difference between H.pylori colonization density grade I and grade II or III(p<0.01).3. There's a significant difference of CD74 expression between H. pylori positive objects and H.pylori negative ones(F=14.571,p<0.01). The comparison of different H. pylori colonizationgroups always showed significant difference (p<0.01) expcept the comparison between grade IIand III(p>0.05).4.P-ERK expression is positively correlated with the H.pylori colonization density(p<0.01,rs=0.581), and CD74 expression is also positively correlated with the H.pylori colonizationdensity(p<0.01, rs=0.539). While the expression of p-ERK and CD74 showed positivecorrelation(p<0.01, rs=0.33 4 ). The result is confirmed by SIMPLE method.Conclusions1.The expression of p-ERK is remarkably higher in GCC comparing to normal mucosa and adjacent nontumorous mucosa , and there's more expression of p-ERK protein in poordifferentiation GCC tissues. The clues indicate p-ERK protein may play a role in the progressionof GCC and may be linked with differentiation.2.It shows a positive correlation between p-ERK expression and H.pylori colonization densitywhich clews the latter may activate the expression of p-ERK.3.The expression of p-ERK,CD74 protein and H.pylori colonization density present positivelycorrelated among each other which hints H.pylori/CD74 pathway may trigger ERK activation.4. H.pylori, p-ERK and CD74 protein are confirmed co-expressed in GCC by using SIMPLEtechnology. On the basis of relevant references, H.pylori might take a participate in GCCprogression by H.pylori-CD74- ERK pathway.
