Objective: To research the relation between serum levels of osteopontin (OPN), vascular endothelial growth factor (VEGF) and acute leukemia (AL) in different periods and groups. In primary untreated AL patients, investigate the relationship between serum levels of OPN, VEGF and peripheral white blood cell (WBC) count, bone marrow blast cell, lactate dehydrogenase (LDH), beta 2-microglobulin (β2-MG). At the same time, investigate the correlation between OPN and VEGF. Further to explore the correlation of OPN and VEGF with the occurrence and development of AL. Through the study, provide the theoretical basis of antiangiogenesis therapy in AL.Methods: Serum levels of OPN and VEGF in 25 primary untreated patients group, 19 complete remission (CR) group, 14 non-remission (NR) group, 11 relapsed patients grooup with AL were measured by enzyme-linked immunosorbent assay (ELISA) to compare with those of 20 healthy persons in control group.Results: 1. Through quantitating serum levels of OPN in different AL groups (primary untreated patients, CR, NR, relapsed patients, normal control group), it is found that the difference in serum levels of OPN between primary untreated patients group and normal control group had the remarkable significance (P<0.01); serum levels of OPN in CR group decreased obviously, and there was no significant difference between CR group and normal control group (P>0.05); serum levels of OPN in relapsed and NR patients were significantly higher than that in the control and CR group (P<0.01).2. Through quantitating serum levels of VEGF in different AL groups (primary untreated patients, CR, NR, relapsed patients, normal control group), it is found that the difference in serum levels of VEGF between primary untreated patients group and normal control group had the remarkable significance (P<0.01); serum levels of VEGF in CR group decreased sharply, and there was no significant difference between CR group and normal control group (P>0.05); serum levels of VEGF in relapsed and NR patients were significantly higher than that in the control and CR group (P<0.01).3. Through analysis serum levels of OPN, VEGF in different type of AL in primary untreated patients, it is found that no significant difference in serum levels of OPN between acute myelogenous leukemia (AML) and acute lymphoblastic leukemia (ALL) group (P>0.05) as the same as serum levels of VEGF.4. Through analysis serum levels of OPN, VEGF in different type of ALL in primary untreated patients, it is found that a significant difference in serum levels of OPN between Philadelphia chromosome-positive (Ph+) and Philadelphia chromosome-negative (Ph-) group (P<0.05), but there was no significant difference in serum levels of VEGF between two groups (P>0.05).5. Through analysis serum levels of OPN and clinically relevant factors in primary untreated patients, it is found that serum levels of OPN uncorrelated with bone marrow blast cell percentage,β2-MG (r=0.03, 0.36, P>0.05), but positively correlated with peripheral WBC count, LDH (r=0.44, 0.45, P<0.05).6. Throug analysis serum levels of VEGF and clinically relevant factors in primary untreated patients, it is found that serum levels of VEGF uncorrelated with bone marrow blast cell percentage (r=0.02, P>0.05), but positively correlated with peripheral WBC count, LDH,β2-MG (r=0.43, 0.52, 0.47, P<0.05).7. Through analysis serum levels of OPN and VEGF in primary untreated patients, it is found that there is a positive correlation between them(r=0.42, P<0.05).Conclusion: OPN and VEGF have a close relationship with the occurrence and development of AL, serum levels of them are associated with AL in different periods. Both of them may be used as indicators of diagnose disease,observation curative effect,understanding of one's illness and prognostic in AL. There is positive correlation between OPN and VEGF, and there are stimulative mechanism of expression and synergy in process of angiogenesis between them. Osteopontin is upregulated by BCR-ABL. Therefore, antiangiogenic therapy of inhibitors targeting OPN and VEGF may represent an effective strategy for treating patients with AL.
