Objective: To investigate the safety of 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors (statins) in patients with acute myocardial infarction. Methods: We screened patients hospitalized in Tongji Hospital between January 2003 and May 2009 with acute myocardial infarction (AMI) and selected 198 cases meeting the inclusion criteria. According to the level of alanine transaminase(ALT)on admission and whether or not receiving statin therapy, all the cases were divided into 3 groups: group A with elevated ALT and received statin therapy; group B with elevated ALT and did not receive statin therapy, group C with ALT normal and received statin therapy. All the patients had received anti-platelet aggregation, anticoagulation, Anti-ischemic drugs treatment. Compare with the liver function after treatment for one week. Result: Total cholesterol and low density lipoprotein cholesterol of patients in group A and group C significantly decreased after receiving statin therapy for One week(P<0.001).Patients with ALT 1.5 times ULN in group A were 24 cases (42.11%).After treatment for 7 days, patients with ALT greater than 1.5 times ULN were 5 patients(8.77%),greater than 3 times ULN were 3 cases (5.26%).Patients with ALT greater than 1.5 times ULN in group B were 15 cases (42.11%).After treatment for 7 days, patients with ALT greater than 1.5 times ULN were 1 patients (8.77%),greater than 3 times ULN were 1 cases (5.26% ).The incidence of ALT greater than 3 times ULN had no difference between group A and B(P=0.51).After treatment, ALT greater than 1.5 times ULN in group C were 4 cases (5.3%), greater than 3 times ULN were 2 cases (3.5%).Compared with group A, incidence of ALT greater than 3 times ULN in group C had no difference(P=0.75).Conclusions: These data suggest that individuals with elevated liver enzymes do not have higher risk for hepatotoxicity from statins.
