Objective:This study aimed at setting a management model to AIDS orphans and establishing associated policies, which basis on the community, according to getting familiar with the situation of the AIDS orphans , physical and mental health, management measures and policies in high incidence of HIV in China.Methods: Six hundred samples of AIDS orphans in Shangcai (Henan Province), and Suizhou (Hubei Province) were collected by cluster sampling method, and were interviewed face to face. The government leaders in charge of health and education were investigated by personal interview or group discussion.Percentage and constituent ratio were used to discripe quantitative data. And chi-square test, multi-factor logistic regression analysis were used in qualitative data, analysis.Results:1. Basic situation: the range of age of the AIDS orphans was from 2 to 15 years. The AIDS orphans were still lack of life skills among 2 to 6 years old, while 67.47% of 7 to 15 years old AIDS orphans had self-regulation skills. The common diseases were infections and gastro-intestinal sickness. There are 92.51% orphans chosing to see a doctor after they felt ill. The problem of food and clothing had been basically solved but nutrient intake was not enough. About fourty percent were brought up by their grandparents / grandma Grandpa, and treated kind by around peole. The ratio of orphans studying in school for free now was 93.13%.There were 60.38% of them supported by society. More than ninety percent teachers were concerned about AIDS orphans, and there is less discrimination from other children to AIDS orphans. 2. Management situation: the local civil affairs departments supplied subsistence allowance for them. They also enjoyed the―two free and one subsidy‖in school. The local government built vocational training schools to solve their employment problems. Parts of antiviral drugs were free for them as they had medical evidence. The local health sector, civil affairs departments and the education sector had AIDS orphans files and interviewed them every month.Conclusion: Although most AIDS orphans were been taken care of by the government and other departments or organizations , their quality of life was poor, life skills was disabled, mental health hadn't been paid enough attention on, and the ways to settle down whom still needed to be improved. The ratio of successfully entering to a higher school of AIDS orphans was high, while the results of them was poor. The supportive funds were deficiency. The government should keep improving the management policy, and exploring a new management model which based on the community, so as to helping and supporting AIDS orphans.
