Objective: The study is to investigate serum AHSG and glycose and lipid metabolism levels during pregnancy and patients with gestational diabetes mellitus. Use the HOMA-A to express the IR of the gestational diabetes. Investigate the effect of AHSG in the pathogenesis of GDM.Method:1.Study: women with primary check and delivery in Guangzhou Maternal and Children's Hospital from 2009-1 to 2009-10. There are 60 cases with GDM which diagnostic criteria based on "Obstetrics and Gynecology" 7th Edition, 80 cases of normal pregnant women (40 in the first,40 in the second ) and 30 cases of normal non-pregnant women during this period. There is no one of the selected women in study has below diseases: hypertension, cardiopathy, nephropathy, diabetic mellitus, hepatopathy, etc, neither blood transfusion or immune therapy, or history of drug abuse.2.Test method:Pick forearm vein blood after limosis the following morning after the selected group live into the hospital. Centrifugalize 20 minutes in the 1000Xg centrifugal machine and then distribute the supernatant into another EP hose, use ELISE to detect plasma AHSG, meanwhile, detect serum, glocuse, insulin, triglyceride, total cholesterol, low density, lipoprotein, high density lipoprotein. Calculate homa-insulin resistance index(HOMA-A). Use above methord to test the healthy non-pregnant women. Picked forearm vein blood after limosis once more during 37-40 weeks, detected serum AHSG by ELISE, to investigate the changing if the serum AHSG level in different weeks. Measure the weight of the neonatal birth. Use the soft ware of SPSS for Windows 11.0 to analyze the data.Results1.The AHSG levels of normal non-pregnant women, normal pregnant women and patients with GDM respectively are as: 130±7.83μg/ml ,145.61±25.57μg/ml, 253.75±49.92μg/ml. The AHSG level in GDM group is obviously higher than NGT group and NP group.2.The HOMA-A in GDM group(3.16±0.54) are the highest in three group. (2.03±0.32) (1.19±0.22) (p<0.01). The HOMA-B(114.31±32.50) in the GDM group are the lowest among the three group. (p<0.01)3.FINS in GDM group (14.26±1.15mmol/L)is the highest among the three groups (p<0.01).There is significantly difference.TG,TC,LDL in the GDM group are higher than that in the control group (p<0.01), HDL in the GDM group is the lowest among the three groups. (p<0.01)4.Serum AHSG level is positively correlated with HOMA-A, TC, TG, LDL-C (r=0.559, 0.306.0.305.0.304, P<0.01),and negatively correlated with,HOMA-B,HDL-L(r=-0.155,-0.596,p<0.01).Multiple regression analysis shows the regression to HDL-L ,QUICK:Y=-3208.185+106.015XFINS -104.525XHDL-L+3268.273 XHOMA-A.5. In GDM group the neonatal birth weight is higher than NGTgroup. (P<0.01)ConclusionThe normal pregnancy exists insulin resistance, gestational diabetes mellitus has stronger IR. GDM women have the performance ofβ-cell dysfunction; Serum AHSG has significantly high IR, and positively correlated with FINS, TG , TC, LDL-C, HOMA-A, may be involved in the formation of IR and associated with glycose and lipid metabolism in GDM.
