Day 2 Versus Day 3 Embryo Transfer: A Prospective Study Of Clinical Outcomes

Objective:To compare the outcome of day 2 and day 3 embryo transfers in infertility couples of late-cleaving embryos and also early-cleaving embryos in China.Methods:This is a retrospective randomized trial which is held in Reproductive Medical Center of Women's Hospital, School of Medicine, Zhejiang University, in Hangzhou, China. A total of 490 women with only late-cleaving embryos for IVF and ICSI cycles including 98 embryo transfers on day 2 and 392 on day 3. And 3010 patients with only early-cleaving embryos transferred were included by certain criteria of which 619 patients in day 2 and 2391 in day 3 group. We set Implantation rate, clinical pregnancy rate, and multiple pregnancy rate as main outcome measure.Results:Clinicl pregnancy rate (33.93% vs.14.29%, p< 0.01) and implantation rate (17.77% vs.9.68%, p<0.01) were significantly higher in the day 3 embryo transfer group compared with day 2, but the multiple pregnancy rate (35.71% vs.15.04%, p< 0.05) was significantly lower onday 3 in patients of late-cleaving embryos transferred. We also observed that in early-cleaving embryos transferred patients there were significant differences in the clinicl pregnancy rate (43.62% vs.49.02%, p<0.05), implantation rate (27.29% vs. 31.35%, p<0.01) between day 2 and day 3, but the multiple pregnancy rate (31.48% vs. 32.51%) was similar.Conclusions:Our results demonstrated that transfering embryos on day 3 could have a better chance of achieving a live delivery than day 2.
