Objective To discuss the effect of different approaches of resection and drainage for hilar cholangiocarinomas(HC).Methods Data of 74 patients with HC treated in our hospital were analyzed retrospectively, including the resection rates, survival rates. Results The 1-, 3- and 5-year cumulative survival rates for patients who underwent resection were 76.60%, 36.20% and 10.60% respectively, which were higher significantly than those of palliative operation group; moreover, the 1-, 3- and 5-year cumulative survival rates for patients who underwent radical resection were 88.90%, 44.40% and 13.90% respectively, which were improved significantly compared with those( 36.40%, 9.10% and 0.00%) in non-R0 resection. After EMBE for patients who were intolerant of resection, liver function was improved at 2 weeks post-operation and the 1-, 3- and 5-year cumulative survival rates for these patients were significantly higher than those of non-R0 resection group.Conclusion With HC, resection is still the major treatment method. R0 resection carries the best effect. Extended radical resection of liver lobes, blood vessels, lymph nodes can prolong survival time. The preferred approach of drainage in palliative operation is EMBE, which is more appropriate than tumor resection for the patients who suffer from serious comorbidities.
