There is a protection mechanism, on the surface of normal bladder mucosa, that can prevent inflammation, bacterial clumps, carcinogenic substances etc from adhering to the bladder mucosa above. For some reason, when this protection mechanism is destroyed, all kinds of inflammatory substances in the urine would worsen the damage of bladder mucosa, causing a series of bladder inflammation. Some scholars have put forward the bladder barrier theory that the tight junction between bladder epithelial cells, specialized cells of the umbrella, contributing to bladder mucosa block up the invasion of bacteria, certain molecules and ions. This barrier is composed of the Glycosaminoglycan (GAG) , which is distributed the surface of the bladder lamina propria layer. Some scholars have conceived to instill the Hyaluronic acid (HA) , heparin and multi-pentosan in the bladder for Interstitial cystitis (IC) patients, to repair damaged GAG layer, recover protective barrier of bladder mucosa, efficacy significantly.Objective:To study the expression of IL-8 protein and EGF in SD rats model of cyclophosphamide-induced cystitis and investigate IL-8 and EGF changes after intravesical instillation with hyaluronic acid. Methods:Rats were randomly divided into 4 groups(A, B, C, D),eight rats in each group. Group A was used as blank control, B as positive control , C as placebo group with physiological saline, D as experimental group with hyaluronic acid. Intraperitoneal CYP were injected in female SD rats of group B,C and D.The rats of group C and D were instillated intravesically with physiological saline and HA respectively,We collected urine for three hours before inducation, after 3 days of CYP injection and after 24 hours of three instillations, Urine specimens obtained from rats kept in metabolic cages, All samples were frozen immediately and stored in the -80℃refrigerator, Subsequently killed all mouse and collected the bladders. Then the levels of IL-8 protein and EGF were analyzed with ELISA in each urine sample. Using conventional staining techniques, we observed the pathological bladder mucosal injury and inflammatory infiltration.Results:Endoscopic pathological changes, Group A were normal basely. Group B,C had mucosal hyperemia, a small amount of bleeding, mucosal edema and inflammatory cells infiltration was noticeable under the bladder epithelium, Group D,scattered in small punctate hemorrhage. Concerning the results of IL-8 and EGF expression, before induction, there were no significant in comparison with four groups(P>0.05). After induction , Group A compared with group B,C and D significantly(P<0.01),B,C and D three groups compared with each other insignificantly .After intravesical instillation, A group, when compared with B, C groups, There was a significant with them (P<0.01), and when compared with D groups,it was not significant (P>0.05), and when compared with C, D groups it showed no significant(P>0.05). Conclusion:The levels of IL-8 protein and EGF elevated in rats of CYP-induced cystitis, After intravesical instillation with hyaluronic acid ,its expressions decreased. There might be objective values in diagnosis of bladder mucosa injury and assessment of therapeutic effect of mucosa repair.
