The Recent Analysis Clinical For The Treatment Of ANFH Of Ficat 2 Stage By Using Tibial Periosteal Transplantation

Objective To analyze the recent clinical effect of the tibial periosteal transplantion for the ANFH of ficat two stage through two methods-the control group (core decompression and bone graft through trochanter) and experimential group (tibial periosteal transplantion and bone graft ).Methods From february 2008 to the february 2010,a series of 40 patients(40 hips) in our hospital were involved in this study.The patients divided into two groups randomly that 20 patients in the control group and 20 patients in the experimental group.The two groups have comparability by comparison of general factors were no significant differences by age,sex and the cause of disease.To rate all patients at three months and six months after operstion.The results were evaluated by the hundred forked method.Results All patients clinical results were satisfactory, there is no complications such as infections, vascular and neurovascular injury. 40 patients were followed up by 3 and 6 months. The pain of the hip area in all patients disappeared or morstly relieved after the operation. The height of the femoral head after the operations was increased in different groups, and the motion of hip joint became better than control group. Comparing to the control group, the scores of clinical manifestation included pain, function,motion of hip, X-ray evaluation and total scores in patients were significantly increased when follow-up after operation, P<0.01.Conclusion There is no one method is suitable for all situations of the femoral head necrosis treatment .Therefore, to explore a new surgical treatment for early femoral head necrosis, to prevent femoral head collapse, delaying joint replacement time, improve the quality of life in patients have a high clinical significance. This operation can be looked at under completely remove necrotic tissue capsule, with variable pressure fresh chancellors bone implanted, necrotic area revascularization, and remove obstacles to prevent further collapse of femoral head. Shin preinstall transplantation can increase the blood supply of the femoral head .This surgical method can be used for the patients of Ficat 2 steage.
