Objective To summarize the incidence, occurred regularity and treated experience by analyzing the cases of malignant glaucoma, thus so as to guide clinical diagnosis and treatment.Methods From January 2005 to December 2009, hospitalized patients in the department of ophthalmology were summarized retrospectively in The First Affiliated Hospital of Fujian Medical University. Cases of glaucoma underwent surgical treatment were investigated emphatically and cases of malignant glaucoma were readed especially. Incidence, clinical category, treatment methods, prognosis of malignant glaucoma were analyzed and summarized.Results 1. There are 1024 all kinds hospitalized cases of glaucoma from January 2005 to December 2009, 892 cases are angle-closure glaucoma, 778 cases were operated, of the total; 936 eyes are operated by filtering procedure. 12 cases (14 eyes) of malignant glaucoma of these cases generated malignant glaucoma. 9 cases (11 eyes) occurred after trabeculectomy of glaucoma, 1 case (1 eye) resulted from other phacoemulsification and IOL implantation, retinal detachment surgery resulted in 1 case of malignant glaucoma with 1 eye, 1 case (1 eye) underwent phacoemulsification, intraocular foldable lens implantation combined with trabeculectomy occurred malignant glaucoma. 1 case of 12 cases (14 eyes) refused surgery and leave the hospital, 11 cases (13 eyes) accepted vitreous suction and anterior chamber reconstruction surgery or cataract surgery and IOL implantation, the anterior chamber of 9 cases (10 eyes) recovered from the operation and their intraocular tension went down to a normal level. The anterior chamber of 1 case (2 eyes) revived but their intraocular tension remained at the higher level, so their intraocular tension were been controlled long term by topical application of drug. 1 case (1 eye) didn't obtain the clinical outcomes postoperatively, then we performed anterior vitrectomy and extirpated the IOL, the intraocular tension and anterior chamber recovered from the second operation.Conclusion 1. Incidence of malignant glaucoma after underdoing any operation for hospitalized patients with angle-closure glaucoma was 1.16% (9/778 cases). 2. The majority of malignant glaucoma is the typical malignant glaucoma. 3. The success of treatment was largely due to vitreous suction and anterior chamber reconstruction surgery or cataract surgery and IOL implantation. Only a minority of the cases were performed anterior vitrectomy to recover the intraocular tension and anterior chamber.
