BackgroundThe brain damage syndrome is an interim diagnosis for the high risk infant with brain damage who has not been clearly diagnosed with cerebral palsy, dysgnosia, language disorders, epilepsy or dystropy, and is commonly used in babies before six-month-old. Because of the development of perinatology and the newborn emergency medicine, the neonatal mortality rate drops considerably and the neonatal survival rate of the premature, very low birth weight infants and the critically ill babies rise gradually. It leads to a trend of ascent in the morbidity of brain damage syndrome. As the development of domestic rehabilitative medicine, rehabilitation workers have pay more attention to the early intervention of brain damage syndrome with the purpose of reducing the formation rate of nervous system sequela.ObjectiveA randomized controlled trial was conducted to objectively assess the clinical efficacy of the Massage of Tongduxingnao and Yishenjianpi and the Massage of Sensory Stimulation in the treatment of brain damage syndrome. The effects of the two groups is compared to see whether there are superiority in early intervention of brain damage syndrome with the Massage of Tongduxingnao and Yishenjianpi.MethodsA randomized controlled trial was conducted.80 infants with BDS in Nanhai Affiliated Maternity and Children's Hospital of Guangzhou University of Chinese Medicine were selected, randomly divided into two groups. The experimental group received the treatment of Massage of Tongduxingnao and Yishenjianpi combined with routine intervention. The control group received Massage of Sensory Stimulation combined with routine intervention. The massage treatment of each time was lasted 30-40 minutes,1 time a day,20 days as a treatment course,3 treatment courses would be taken, with 10 days rest between each course. The Gross Motor Function Measure and the Gesell Developmental Scales were tested before, right after and 6 months after the treatment designed. All data would be analyzed by SPSS 13.0.Results1. The children's development quotients (DQ) in both groups are improved. The Massage of Tongduxingnao and Yishenjianpi has a significant superiority in improving the DQ of gross motor function, social adaptation and social intercourse tested by Gesell Developmental Scales compared with the Massage of Sensory Stimulation(P< 0.05), but no discrepancy in the fine movement DQ and linguistic DQ(P>0.05).2. The scores of A, B and C areas (GMFM) are higher when tested right after the treatment and 6 months after the treatment. After the treatment, the score of A area in the treatment group is higher than the control group (P>0.05), but no significant different exist 6 months later (P>0.05). The scores of B and C areas in the treatment group are higher than the control group 6 months after the treatment (P<0.05)3. In the treatment groups, there are 17 cases with abnormal brainstem auditory evoked potential (BAEP), and 16 (94.1%) cases recover 6 months after the treatment;27 cases with abnormal visual evoked potential (VEP), and 25(92.6%) cases recover;35 cases with abnormal trancranial Doppler(TCD),and 11 of them recover. In the control group,12(75%) of the 16 cases with abnormal BAEP return to normal,20 (76.9%) of the 26 cases with abnormal VEP return to normal, and 10 of the 33 cases with abnormal TCD return to normal.ConclusionThe Massage of Tongduxingnao and Yishenjianpi and the Massage of Sensory Stimulation can promote the developmental level of the intelligence and gross motor function, and the former has a significant superiority in improving the DQ of gross motor function, social adaptation and social intercourse. The 6 months follow-up shows that Massage of Tongduxingnao and Yishenjianpi has a stable curative effect. The two kind of massage have the same effect on ameliorating the language ability and fine motor function.
