Study Of The Mechanism Of Eleng Capsule In Intervening Endometrial Adhesion Function In Endometriosis

BackgroundEndometriosis is a common disease in the women of childbearing age, its incidence increases year by year, and there is getting younger and younger. Endometriosis is often associated with pelvic pain and infertility. And the characteristic of easy relapse after treatment has become a research hotspot in recent years. Currently the disease has been treated by various of methods, such as drugs and surgery. But they did not really play the long-term role of eliminating symptoms and cure. Since the disease mostly occurs in the women of childbearing, conservative surgery has become the most important method of treatments. Conservative surgery reserves ovarian tissue, so it relapses easily. Although the modern medicine has achieved a certain effect on treating the endometriosis by medical therapy before and after surgery, the high relapse rate after surgery that always a difficult problem puzzles gynecologist. Therefore, the further exploration of more practical and reasonable measures for Integrative Medicine has important practical significance.ObjectivesTo research the effect of application of Eleng Capsule to treat and prevent relapse in endometriosis from clinical aspect, detecting the level of sICAM-1,IL-6,CA125,EmAb in serum, to explore the action of Eleng Capsule in intervening the disease. Try to form the point of huoxuehuayu anti-adhesion of ectopic endometrium, observing the effect of Eleng Capsule on treating and preventing relapse in endometriosis, and explain the possible mechanism. Then clarify the scientific content of huoxuehuayu on treating endometriosis, which is on it intervenes the adhesion of ectopic endometrium.Methods Sixty patients with endometriosis who were treated by conservative surgery were recruited as the endometriosis group, and divided randomly into tow groups, as treatment group and control group, thirty patients in each group. According to the stages of endometriosis of revised American Fetility Society, endometriosis group were divided into stageⅠ,stageⅡ,staeⅢand stageⅣ. During the same period, selected thirty patients who were treated by laparoscopic surgery but non-endometriosis, as non-endometriosis group. Treatment group were treated by conservative surgery, taking Eleng Capsules three menstrual cycles after surgery,6 tablets per time,3 times per day. Control group were also treated by conservative surgery, taking Gestrinone Capsule three months after surgery,2.5mg per time,2 times per week. Treatment group and control group were determined the serum levels of CA125,EMAb,liver function 3,renal function 3 before surgery and the forth month after surgery (the fifth to seventh day in menstrual cycle). At the same time, the tow groups were extracted peripheral blood 5ml.The non-endometriosis group were also extracted peripheral blood 5ml before surgery.The peripheral blood were separated, extracted the serum placed in -70℃refrigerator, then determined the serum levels of sICAM-1 and IL-6 by ELISA. The changes of symptoms and signs were recorded before and after treatment.Result1. In endometriosis group, the serum levels of sICAM-1,IL-6 before treatment were significantly different from non-endometriosis group's (P<0.01).It Shows that the serum levels of sICAM-1,IL-6 in endometriosis patients were significantly higher than those of non-endometriosis.2. The serum levels of sICAM-1,IL-6 in stageⅢandⅣof endometriosis were higher than those in stageⅠandⅡ.It shows that the serum levels of sICAM-1,IL-6 of endometriosis patients increased with the aggravation of the disease.3. After treatment, the serum levels of sICAM-1,IL-6,CA125 in treatment group were significantly lower (P<0.05), the negative rate of EMAb was 66.67%(P<0.05), dysmenorrhea significantly alleviate, the effect was similar with control group's.4. In the third and sixth month of postoperation, compare with relapse rate, there were no significant differences between treatment group and control group(P>0.05). It shows that Eleng Capsule in reducing endometriosis patients' s relapse rate of postoperation in recent period, was similar with gestrinone.5. During the period of treatment, no one appeared adverse effects such as liver and kidney dysfunction in treatment group. But control group with varying degrees of elevated alanine aminotransferase, irregular vaginal bleeding and other adverse reactions.ConclusionThe serum levels of sICAM-1,IL-6 in endometriosis patients were significantly higher than those of non-endometriosis,and increased with the aggravation of the disease. Eleng Capsule treated the endometriosis patients of postoperation, can relieve dysmenorrhea, lower postoperative recent relapse rate, possibly by down-regulating serum levels of sICAM-1,IL-6,CA125 and EMAb.The effect was similar with gestrinone. But Eleng Capsule has no side effects such as liver and kidney dysfunction, safe, effective and worth promoting.
