ObjectiveDiabetic nephropathy (DN) is one of the common complication of diabetes. The pathogenesis has not yet fully clarified. For the lack of effective treatments, DN is an important cause to death. In this thesis, acupuncture prescription is formulated by means of retrieval from the acupuncture and massage therapy for diabetes in " Encyclopedia of Traditional Chinese Medicine" and analysis of the formula and acupoint characteristics and systematical clinical research of diabetes nephrosis patient to investigate the mechanisms of acupuncture for diabetes and provide the theoretic basis and preliminary effects about clinical treatment.MethodsThe acupuncture prescription applied to clinical is formulated by means of retrieval from the acupuncture and massage therapy for diabetes in "Encyclopedia of Traditional Chinese Medicine" and statistical analysis of the formula and acupoint characteristics.53 diabetes nephrosis patients were divided into the acupuncture group and control group. The acupuncture group was given acupuncture treatment every two days besides basic treatment for 6 weeks. The change of symptoms between two groups before and after the treatment was observed. Serum fasting blood glucose, creatinine, blood urea nitrogen, glycosylated hemoglobin, blood fat and blood rheology were measured. The data was analysed with t-test, paired t-test, chi-square test, Fisher probabilities method.Resultsacupuncture prescription statistics1. Total 50 classics record acupuncture and massage in "Encyclopedia of Traditional Chinese Medicine". The records of acupuncture treatment in "Xiao ke","Shen xiao","Xiao shen","San xiao","Shen lao","Guan ge","Neixiao" etc, total 2192. Acupuncture prescription is formulated according to the analysis of the formula and acupoint characteristics in this classic and then apply to clinical.Clinical curative effect1. Define inclusion and exclusion solution, and randomly divide the 53 patients into 2 groups; acupuncture group with 28 patients and control group with the 25 patients. Symptoms and clinical curative effect showed significant difference between two groups. The acupuncture group was better than control group (P< 0.01) in improving the fatigue, soreness of waist, numbness pain, spontaneous sweating. Concerning the clinical curative effect,8 cases in acupuncture group were remarkably effective,16 cases were effective,4 cases were ineffective, the effective rate was 85.7%. In control group,5 cases were remarkably effective,9 cases were effective,11 cases were ineffective, the effective rate was 56.0%.2. Serum testsa. Fasting blood sugar (FBG):FBG were reduced both in acupuncture group and control group before and after treatment (P< 0.01). And the effect showed significant difference between two groups. (P< 0.01).b. Blood urea nitrogen (BUN):In acupuncture group, BUN decreased evidently (P< 0.01) while in control group it was not obviously reduced (P> 0.05). The effect showed significant difference between two groups. (P< 0.01).c. Serum creatinine (Scr):Scr reduced remarkably in acupuncture group (P< 0.05), while in control group it was not obviously decreased (P> 0.05). The effect showed significant difference between two groups (P< 0.01).d. Glycosylated hemoglobin (HbAlc):HbAlc were reduced both in two groups before and after treatment (P< 0.01). And the effect showed significant difference between two groups. (P< 0.01).e. Microalbumin (mAlb):MAlb reduced evidently in acupuncture group (P< 0.01), while in control group it was not obviously decreased (P> 0.05). The effect showed significant difference between two groups (P< 0.01).f. Blood fat:Total cholesterol (TC), Triglyceride (TG), High density lipoprotein cholesterol (HDL-C) in control group showed no significant difference between before and after treatment (P> 0.05). While in acupuncture group, TC, TG, HDL —C were decreased (P< 0.01). The difference was statistically significant between two groups (P< 0.01).g. Hemorheology:Whole blood viscosity, plasma viscosity and fibrinogen were decreased obviously (P< 0.01). The regulating action of whole blood viscosity and fibrinogen showed significant difference between two groups (P< 0.01).ConclusionThe applying of acupuncture prescription to clinical retrieved from the classics improves the clinical symptoms and reduce blood sugar, serum creatinine, blood urea nitrogen, glycosylated hemoglobin, microalbumin. And the treatment is more effective in acupuncture group than in control group. It is probably concerned with the function of acupuncture in reducing blood fat, improving hemorheology and changing the high sticky state of body.
