ã€Objective】To analyze the effectiveness and safety of acupuncture and moxibustion with heart and gallbladder thesis on obsessive-compulsive disorder (OCD).ã€Methods】A clinical survey on acupuncture treating OCD was conducted in Guangdong Provincial Hospital.70 confirmed OCD cases diagnosed by CCMD-3 were included and divided into two groups:Treatment group (n=35, treated with heart and gallbladder thesis) and Control group (n=35, treated with liver thesis). Both groups were randomized by PEMS 3.1 software and received integrated treatment of acupuncture, moxibustion and subcutaneous needle. In the treatment group, patients firstly had acupuncture on acupoints of Baihui (DU20), Yintang(EX-HN3), Waiguan(SJ5) and Zulinqi(GB41), after 30 minutes,5 times of grain-sized moxa cones were burned on Geshu(BL17), Danshu (BL19) and Zuqiaoyin(GB44), at last subcutaneous needles were embedded on Xinshu (BL 15) and Danshu (BL19) for 3 days. In the control group, acupoints of acupuncture changed to Baihui (DU20), Yintang(EX-HN3), Hegu(LI4), Taichong(LR3), and moxa cones were burned only on Geshu(BL17) and Danshu (BL19). The treatment of both groups had been last 3 months with 2 times per week (or had been treated over 20 times), with over 2days of interval time. The therapeutic effects were assessed by Yale-Brown Obsessive Compulsive Scale(Y-BOCS) and Hamilton Rating Scale for Depression (HAMD) at baseline and after 3 months'treatment. The data were analyzed by SPSS 16.0 software and processed with t test andχ2 test.ã€Results】 The age, sex, disease course, education, past treatment, Y-BOCS score and HAMD score at baseline had no statistical significance, that is, two groups had comparability. Y-BOCS scores and HAMD scores after treatment in two groups were lower than scores at baseline, which had statistical significance (P<0.05). It indicated that the OCD symptoms and depression symptoms of all patients had been improved after intervention. The proportion of total effectiveness by Y-BOCS of treatment group (87.9%) was higher than control group (62.5%) with statistical significance (P<0.05), which indicated that for OCD symptoms using acupuncture with heart and gallbladder thesis was more effective than with liver thesis. However, the proportion of total effectiveness by HAMD of control group (93.8%) was higher than treatment group (72.7%) with statistical significance (P<0.05), which suggested that for depression symptoms of OCD patients using acupuncture with liver thesis was more effective than with heart and gallbladder thesis. And during the whole research process, no adverse reaction happened except 1 case dropped out the treatment for unbearable pain from acupuncture. That is to say, acupuncture was a safe treatment and patients should be told the points of attention before acupuncture.[Conclusion]Acupuncture with heart and gallbladder thesis or liver thesis were clinically effective for OCD patients, while the former one was more effective. As for OCD symptoms, the therapeutic effect acupuncture with heart and gallbladder thesis was better than with liver thesis. As for depression symptoms of OCD patients, the therapeutic effect acupuncture with liver thesis was better than with heart and gallbladder thesis. |
