| BackgroundWith the increasing aging population in China, The incidence of osteoporosis is getter higher. Osteoporosis can lead to vertebral compression fractures easily. After the traditional conservative treatment, patients need a long time bed rest. It may cause a variety of complications and increase osteoporosis. Further more, during the fracture healing, Patients should also bear the pain caused by fractures for a long time. As a new treatment, PVP and PKP can make a significant analgesic effect for most patients. Therefore, they have very broad application prospects. However, a few patients are still not satisfied enough or have residual pain after surgery in a short time after the treatment. There is no literature on statistical studies of the related factors for the not ideal postoperative pain relief.ObjectiveStudy the effectives Of PVP and PKP in both 3 days and 3 months after surgery and discuss different factors related to different pain relief rate after surgery.MethodsSelect 144 patients of osteoporotic vertebral compression fractures after PVP or PKP surgery treatment in our department from January 2009 to December 2010. Rate a VAS score in the preoperative, postoperative 3 days and 3 months after surgery. With SPSS11.5 Statistical package, analyze the single factor first. By the X2 test, analyze the factors related to pain relief rate after surgery, such as age, gender, whether at their own expense, the number of surgical vertebral, whether limited painful area, bone mineral density, with or without history of trauma, Kyphosis angle, the amount of bone cement, course of disease, whether a history of previous vertebral fractures, whether to use hormones for other diseases, leaks, surgical methods, whether a new fracture. Remove no statistically significant variables one by one. On this basis, using multivariate logistic regression, analyze the factors related to pain relief rate.ResultIn postoperative 3 days, VAS scores of patients from preoperative 7.3±2.0 reduce to 2.8±1.9 (p<0.05). And the VAS scores reduce to 1.8±2.0 in 3 months after surgery. It is significantly different from 3 days'(p<0.05). Single factor analysis related to the vertebral body enhancement surgery indicate that pain relief rate 3 days after surgery is related to the number of surgical vertebral, whether limited painful area, bone mineral density, whether to use hormones for other diseases, the amount of bone cement, leaks, and surgical methods significantly (P<0.05). Logistic regression analysis shows that the number of surgical vertebral, whether limited painful area, whether to use hormones for other diseases, the amount of bone cement, leaks, and surgical methods mainly affect pain relief rate (P>0.05). Single factor analysis show that 3 months pain relief rate after surgery is significantly related to whether limited painful area, bone mineral density, course of disease, surgical methods, Kyphosis angle and whether there is a new fracture (P<0.05). Logistic regression analysis shows that bone mineral density, course of disease, Kyphosis angle and whether there is a new fracture mainly affect pain relief rate (P>0.05).ConclusionIn the case of no contraindications, it is good for the patients to perform surgery as soon as possible. It should be noted in the postoperative comprehensive treatment of osteoporosis. |
