Objective LPN (laparoscopic partial nephrectomy) is becoming more and more popular to real kidney tumors. Ischemia-reperfusion Injury (IRI) is not absolutely avoided.How to release IRI become a clinic Gordian knot. To discuss the protect effect of ischemic preconditioning (IPC) arising from arterial occlusion preconditioning(AOP) and 15mmHg CO2 pneumoperitoneum preconditioning(PP) upon rats solitary renal function.Methods Forty male Wistar rats were distributed to four treatment groups randomly and equally,they are sham group (Sham),ischemic reperfusion group (IR),arterial occlusion preconditioning group (AOP) and 15mmHg CO2 pneumoperitoneum preconditioning group (PP). All rats are removed the right kidney after a week, The model referring to solitary kidney rat was established. Two weeks later, sham group were only isolated the left kidney artery;IR group were subjected to 45minutes of left renal ischemia without any preconditioning;AOP group were subjected to 45minutes of left renal ischemia after underling arterial occlusion preconditioning twice, the pattern of IPC was 10 minutes of left kidney arterial opening followed immediately by 10 minutes of occlusioning. PP group were subjected to 45 minutes of left renal ischemia after the pressure 15mmHg CO2 pneumoperitoneum preconditioning twice the pattern of IPC was 10 minutes of insufflation followed immediately by lOminutes of deflation. After 24h of reperfusion the rats were sacrificed and left kidneys were excised. Blood urea nitrogen (BUN), serum creatinine (Cre), kidney injury molecule-1(Kim-1)and neutrophil glatinase-associated lipocalin (NGAL) were tested. Besides, kidney samples were tested malondialdehyde (MDA),superoxide dismutase (SOD), histopathologic differences of kidney tissue were assessed by experienced pathologists.Results Kidneys of IR,AOP and PP group had severe IRI,compared with Sham group, there were significant difference of renal injury in (P<0.05), but AOP group and PP group had more optimistic data in biochemical indicators and histopathologic outcome (P<0.05).Meanwhile, statistical significance was found group in PP group (P< 0.05)Conclusions We found 45min of clamping renal pedicle resulted in severe IRI in IR group comparing to sham group,IPC arising from arterial occlusion preconditionimng and pneumoperitoneum preconditioning relieve such a injury,and IPC arising from pneumoperitoneum preconditioning can improve anti-oxidation capability of solitary kidney against IRI.We thought IPC of 15mmHg CO2 PP could become one suitable method against IRI.
