Objective:Maintenance hemodialysis is important treatment methods of patients with end-stage renal disease, although the medical has been highly developed, maintenance hemodialysis patients'quality of life and survival remains to be improved. Nutritional status and psychological state of maintenance hemodialysis patients are quality of life and survival of important factors.In this study, we examined the nutritional status and the psychological state of the patients of our Blood Purification Center to know the nutritional and psychological status of the patients,and estimate the relationship between the nutritional status, the psychological status and the different methods of nutrition assessment,so we can implement measures early and improve the quality of life and survival.Methods:A total of 84 ESRD patients with maintenance hemodialysis were recruited and we got the consent form all the patients before the study began. Through the Questionnaire, physical examination, laboratory measurements,we used MQSGA,MIS,BIA to evaluate the nutrition status and used SAS and SDS to assess the psychological status of the patients. Data were processed by SPSS 17.0 statistics software package.Measurement data were shown by(x±s),the comparison between two groups is t-test. The test standard was bilateralα=0.05.Results:1. Nutritional statusNutritional status mean of MQSGA scores of patients was 14.76±5.17, the rate of malnutrition is 75.0%,there was no significant difference between men and women. Mean of MQSGA scores of patients was 8.17±4.06, there was no significant difference between men and women.only one (1.2%)out of 84 patients had a body fat percentage that was below normal, based on the BIA. There was significant diffenent between men and women of the body fat (P<0.05)2.Psychological statusThe SDS and SAS scores of the patients were 52.92±7.87,46.79±7.98, the domestic norm of Depression (1340 cases) SDS score was 41.88±10.57, the domestic norm of Anxiety (1158 cases) SAS scores was 37.23±12.59.By the T test,the scores of the 84 patients were high than the domestic norm. There was no significant difference between men and women of SAS and SDS score. The scores of SAS and SDS were positively correlated(r=0.842, r2=0.709, p<0.01).3.Analysis of correlation between nutritional status and psychological stateThe SDS scores were positively correlated with the values of MQSGA,MIS and BMI, the same as the SAS scores.4.the comparison between the results of different methods84 patients were divided into two groups according to MQSGA score-nutrition group(MQSGA score≤10) and malnourished group (MQSGA score 11-35 points), compare the MIS scores of two groups by t test. P<0.01, there were significant differences between two groups's MIS scores.To exclude the gender effects on the body fat percentage, we divided men into two groups by MQSGA score (nutrition group and malnourished group), as well as women. Compare the body fat percentage between different groups in men and women. The p were both more than 0.05 in men and women, there was no significant differences between nutrition group and malnourished group in body fat percentage.The MIS scores were positively correlated with the values of MQSGA (r=0.924, r2 = 0.854, p<0.01). Furthermore, the percentages of body fat in individual patients as determined by the BIA tended to be only negatively associated with the MQSGA and MIS scores. However, these correlations were of insignificant difference (r=-0.128, r2=0.016, p=0.245,p>0.05 for MQSGA; r=-0.213, r2=0.045, p=0.052, p>0.05 for MIS).Conclusion:(1)There was a high incidence of malnutrition in maintenance hemodialysis patients.(2)The incidence of depression and anxiety was high in maintenance hemodialysi patients.(3)The nutritional status of maintenance hemodialysis patients were positively correlated with the psychological state.(4)In our study, the percentages of body fat in individual patients as determined by the BIA tended to be only negatively associated with the MQSGA and MIS scores.Our data indicate that MIS, but not BIA is a comprehensive method for the evaluation of malnutrition in patients with maintenance hemodialysis.
