Objective:Through detecting myocardial markers(CK-MB,α-HBDF,cTnI Mb),C-reactive protein and abnormal rate of cardiogram in infants with ordinary pneumonia, severe pneumonia and healthy physically, and comparing the above myocardial markers and CRP' levels in severe pneumonia acute period with which in severe pneumonia recovery period, analyze comprehensively and dynamically the relation between the levels of myocardial markers, CRP and infants'pneumonia's occurrence, development, outcome and prognosis, in order to let local pediatricians diagnose infants'pneumonia timely and treat it reasonably. Methods:Measuring the levels of serum myocardial markers (CK-MB,α-HBDH,cTnI,Mb) and CRP in 60 healthy infants,60 infants with ordinary pneumonia and 60 infants with severe pneumonia, meanwhile detecting auxiliarily conventional cardiogram. The results are analyzed by SPSS 11.5 statistic software, One-Way ANOVA is used in the comparision among control group, ordinary group and severe group, then using q-test or nonparametric test to respectively analyze statistics in two groups of the three groups. Analyzing the ordinary and severe pneumonia'influence on myocardium and infectious degrees. Using paired-samples t test or rank sum test to compare the levels of myocardial markers and CRP in severe pneumonia acute period with which in recovery period. According to the results, it can access the curative effect and prognosis. The results of cardiogram are used X2test to compare the abnormal rate among three groups.Results:1,The comparison of the myocardial markers:There is significant difference among the severe group, ordinary group and control group in analyzing the levels of CK-MB,α-HBDH,cTnI,Mb(P<0.05), and the results of severe group and ordinary group are both higher than that of control group (P<0.0167), the results of severe group are higher than that of control group (P<0.0167). The level of Mb has significant difference among above three groups (P<0.05), but there is no obvious difference of Mb between ordinary group and control group (P>0.0167), the differences between severe and control groups, severe and ordinary groups are both significant statistically (P<0.0167). The levels of above myocardial markers in severe pneumonia acute period are higher than that in recovery period (P<0.05).2,The comparison of the C-reactive protein:There is significant difference among the severe group, ordinary group and control group in analyzing the levels of C-reactive protein (P<0.05), and the results of severe group and ordinary group are both higher than that of control group (P<0.0167), the results of severe group are higher than that of ordinary group (P<0.0167). The level of CRP in severe pneumonia acute period is higher than that in recovery period (P<0.05).3,The comparison of cardiogram's abnormal rate:The cardiogram's abnormal rates of severe group and ordinary group are both higher than that of control group (P<0.05), the result of severe group is higher than that ordinary group (P<0.05).Conclution:1,The levels of myocardial markers (CK-MB,α-HBDH,cTnI,Mb) have intimate relation with infectious degree in pneumonia. The infection is more severe, the results would rise more significantly.2,The cardiogram's abnormal rates among the three groups have the same ascend tendency with the myocardial markers'levels among the three groups. Combining above myocardial markers with cardiogram detects can effectively monitor the myocardium injury with infants' pneumonia, and provide the theoretical basement for clinical physicians whether they use medicines to nurture myocardium in treating pneumonia, can observe disease outcome and access prognosis.3,Combining with measuring CRP can effectively reflect infectious serious degree, and provide guidance for pediatricians to use antibiotics reasonably, and access treatment effects.
