ObjectiveTo observe the activity change and its clinical significance of myocardial injurymarkers when adults with severe pneumonia.MethodsA total of118patiens were divite into three groups according to clinicaldiagnosis:forty Usual pneumonia patiens,forty severe pneumonia patiens andthirty-eight Normal Control.To detect the myocardial injury markers such asAspartate Aminotransfer ase、Hydroxybutyrate Dehydrogen ase、Creatine Kinase、Creatine kinase isoenzyme、 Lactate Dehydrogen ase and Cardiac Troponin Irespectively.AND then to Comparison the activity of myocardial injury markersbetween the three groups. After a week,detect the myocardial injury markers ofsevere pneumonia patiens again. We also divite the severe pneumonia patiens intotwo subgroup according to whether the Cardiac Troponin I is increased or not,andthen comparison the age,APACHEII score and prognosis of this two group.Results1. The rate of abnormal myocardial injury markers in the cases with severepneumonia was significantly higher than that in the cases with usual pneumonia (P<0.01).2. The activity of myocardial injury markers in the cases with severepneumonia was significantly higher than that in the cases with usual pneumonia andnormal control (P<0.01).But there were not significantly difference of myocardialinjury markers activity between the usual pneumonia group and normal control(P>0.05).3. After a week’s treatment, the activity of myocardial injury markers in thecases with severe pneumonia was significantly decline than before (P<0.01).4. The rate of poor prognosis such as electrocardiographic abnormality, heart failure, shock and multiple organ dysfunction syndrome in increased cardiac troponinI subgroup were significantly higher than that in normal cardiac troponin I subgroup(P<0.05).But here were not significantly difference of Mortality rate,age andAPACHEII score between the two subgroup(p>0.05).Conclusion:The myocardial injury markers activity can be increased in different degreeswhen adults with severe pneumonia, The cardiac troponin I can serves as a severepneumonia combined biochemical indicator of the early diagnosis of myocardialdamage.
