ObjectTo analyze the clinical characteristics of the hilar cholangiocarcinoma, discuss the clinical diagnosis, treatment and prognosis. MethodsThe clinical data such as sex, age, clinical symptoms, Bismuth classification, modified T-staging, therapeutic schemes, survival time, etc of 110 patients with hilar cholangiocarcinoma who received treatment from 2002.2 to 2010.10 in Xiangya 2nd Hospital Central South University were analyzed. ResultsIn patients treated as hilar cholangiocarcinoma in past 8 years,the number of female and male patients was 55 and 55 respectively. The age range from 23 to 83 with median age was 55.38, and patients predominantly falled in the age range of 55-65. The painless evolutional jaundice was the most common clinical manifestations of the hilar cholangiocarcinoma. The average TBIL was 231.51 umol/L, while the average DBIL was 167.19 umol/L.85 cases got surgical treatment,30 cases of radical resection,4 cases of palliative resection,50 cases of surgical internal or external drainage. The chief pathology type was moderately differentiated adenocarcinoma. Overall survival was 49.4% at half year,22.2% at 1 years and 8.6% at 2 years Conclusion1.Hilar cholangiocarcinoma is more common in elderly between 55-65 years old and moderately differentiated adenocarcinoma, onset with painless evolutional jaundice at the late stage.2.Therapeutic schemes, pathology type, modified T-staging and postoperative chemotherapy are independent prognostic factors.3.Surgical treatment can prolong the survival time of the patients with hilar cholangiocarcinoma, radical resection is the preferred method.4.Both of Bismuth classification and modified T-staging can assess the survival of HCC, while the modified T-staging can assess the resectability better.
