Purpose To view clinical effects of Ning xue yin on children with refractory ITP,and the effects of Ning xue yin on platelet system and immune function of mice in idiopathic thrombocytopenic purpura (ITP) model..To try to study the mechanism of Ning xue yin treatment of ITP. At the same time ,to provide experiment basis for applicating of Ning xue yin in clinic and exploiting of new medicine.Methods There are clinical observation and experimental research. Clinical observation:40 children with refractory ITP were randomly divided into two groups:intervention group of Ning xue yin with 23 and hormone group with 17. The intervention group was treated with Ning xue yin,combined with prednisone and the hormone group was treated with prednisone. The children's skin purpura and other hemorrhagic symptoms, platelet count,antibody against platelets level,and long-term efficacy were observed. Experimental research: The ITP model was established and the mice were divided in to 5 groups: the model group, the high-dose Nin xue yin group, the low-dose Nin xue yin group, the platelet capsule group and the control group. Drugs were administrated in each group 7 days after the model established. Platelet count measuring were performed before the model to be established,7 days after the model established, and 8 days after drug administration. Eye blood was extracted 8 days after drug administration. Double–antibody sandwich enzyme-linked immunosorbent assay to detect the associated antibodies in peripheral blood platelet. After death, megakaryocyte and giant middle plate were accounted by getting marrow from the femur bone; spleen was weighed and its coefficient was calculated. The change of pathology of spleen was observed under the microscope.Results Clinical observation :the improvement of bleeding symptom,raise platelet count,reduce the level of PAIgG,total efficiency and long-term efficacy of the intervention group were significantly better than those of the hormone group after the treatment. Significant difference was found between 2 groups(P<0.05). Experimental research:①Compared with the normal group,the counts of blood platelet of the model mice decreased obviously, the counts of megakaryocyte in bone marrow of the model group mice increased,but the megakayocyte which can produce blood platelet decreased, the level of platelet-associated antibodies was highered ,the spleen was enlarged,and the megakaryocyte in splenic red pulp increased. The results of experiment were in accordance with clinical manifestation;②In high-dose Nin xue yin group, the weight and platelet count were increased. Megakaryocyte count was reduced but giant plate count was increased. The level of platelet-associated antibodies was lowered. Spleen weight and spleen factor were decreased,significantly different from those of control group (P <0.05).At the same time,megakaryocyte in splenic red pulp was significantly decreased compared with control group;③In platelet capsule group, the weight and platelet count were increased. Megakaryocyte count was reduced but giant plate count was increased, significantly different from those of control group (P <0.05);④In low-dose Nin xue yin group, the platelet system has improved, but no significant difference observed compared with the control group (P >0.05).Conclusion According to the experiment results above mentioned,the conclusion of the study are as the follows:①The therapy of hormone combined with Ning xue yin has good curative effect on the treatment of children with refractory ITP. Its adverse reactions are little and long-term effect is superior. It is worth promoting in clinic.;②Confirmed by experimental study ,the therapy mechanism of Ning xue yin may be protecting immune organ—spleen, reducing the level of PAIg, reducing the destruction of platelets,and promoting megakaryocyte maturation,which would make patelet produce increased. Its efficacy is closely related to dose.
