Objective To understand cross border situation, AIDS knowledge, highly risky behavior, AIDS prevention service in China and HIV prevalence of drug users from Myanmar, to explore HIV risky factors both infection and transmission in China through the research of drug users from Myanmar in China-Myanmar border area in order to take on effective strategies and measures for this group to control and prevent AIDS.Methods The study sample was drug users from Myanmar in Forced Detox. Center and Farm Detox. Center by census, while sample in Jiegao yard was recruited by snowball sampling. HIV antibody test were done. Epidata was used to input datum, and double check was used to test the quality of data. Besides, group interviews of villagers were done to reflect the cross border information of drug users in both Chinese villagers and Myanmar villagers from another point of view.Results (1) Among 406 subjects, male took for 93.3%; their mean age was 27.00±11.00; the minority group of Dai took for 24.4%, Burmese 23.4%; illerature and elementary took for 64.5%; fanner took for 40.1%.(2) Twenty five percent of them came from Muse, and fifty nine percent of them was in Ruili in China. The top one reason of crossing border was going to find jobs, taking for 70.9%, while crossing for buying drugs made a proportion of 5.4%. There were 213 subjects who frequently came cross border area both Myanmar and China, taking for 52.5%. Among 213 subjects, the crossing frequency of one to five per month took for 71.8%. Subjects in Jiegao yard had most Chinese friends who used drugs in China. There was 38.6% of them who had ten to fifteen Chinese friends who used drugs in China.(3) Among 406 subjects, there were 242 subjects who were injection drug users. Among 242 IDU. fourty five percent of them injected drugs in China each times one month ago. The percentage of sharing needles and injection water was respectively 34.3%,31.4% one month ago. Those who shared needles and injection water with both IDUs from Myanmar and IDUs from China were most in Forced Detox. Center. The percentage was respectively 18.2%.35.7%.(4) Among 263 subjects who had sex one month ago, there were 47.9% of them who had more than two sexual partners. Only thirty percent of them used condoms last sex intercourse. The percentageof having sex with female sex worker (FSW) from both Myanmar and China was 39.1% among 248 male drug users from Myanmar.(5) HIV positive rate was 19.2% among 318 subjects who had been tested HIV antibody. Logistic regression show that HIV influenced by crossing border for buying drug (OR=8.28), Chinese friends who used drugs (OR=1.60). Among 242 IDUs, HIV positive rate was 26.7%, and their HIV risky factors was sharing injection water with both IDUs from Myanmar and IDUs from China one month ago (OR=3.67), sharing used needles with both IDUs from Myanmar and IDUs from Chian (OR=3.67), sharing both injection water and used needles (OR=3.50). Among 263 subjects who had sex one month ago, their HIV positive rate was 18.5%, and their HIV risky factor was having sex with both FSW from China and FSW from Myanmar one month ago (OR=2.12).(6) Among 406 subjects, there was 43.8% of them correctly answered this question that having sex with only one sexual partner can reduce AIDS transmission. Subjects in Jiegao yard understood the worst AIDS knowledge. There was only 18.2% of them correctly answered the two question that using condoms can reduce AIDS transmission and having sex with only one sexual partners can reduce AIDS transmission. Subjects below primary school had worse AIDS knowledge (OR=0.44) compared with subjects above high school. Subjects in Forced Detox. Center and Farm Detox. Center had better AIDS knowledge (OR=1.89, OR=2.13) compared with subjects in Jiegao yard. Higher AIDS scores, more AIDS relative services.(7) Thirty three percent of them didn't receive any AIDS service in China. Subjects in Forced Detox. Center and Farm Detox. Center received worse AIDS service than subjects in Jiegao yard (OR=0.28. OR=0.12). Subjects who had 0-3 and 4-6 AIDS scores received less AIDS service compared with subjects who had 7-9 AIDS scores (OR=0.21, OR=0.43).Conclusion Drug users from Myanmar mainly cross by informal routes, and their crossing border times were highly frequent. Most of them crossed for finding jobs. They also had some Chinese friends who use drugs. Sharing injection water and used needles with both IDUs from China and IDUs from Myanmar and having sex with both FSW from China and FSW from Myanmar increase the risk of HIV infection.
