[Objective] To evaluate magnetic resonance cholangiopancreatography (MRCP) compared with endoscopic retrograde cholangiopancreatography (ERCP) in the diagnosis of pancreaticobiliary diseases.[method] Retrospective reaserch in June 2008 to March 2011 total of 93 cases with abdominal pain, jaundice and other suspected pancreaticobiliary diseases underwent both MRCP and ERCP in the First affiliated Hospital of Kunming Medical College. Comparison of the two inspection methods that the situation of different diseases. MR imaging equipment:GE 1.5T SIGNA superconducting magnetic resonance imager, using of SSfSE-MRCP and BH Thick slab MRCP sequences in detection of retinal arteriospasm. ERCP imaging equipment:Olypus JF-240 electronic duodenoscopy and Toshiba digital fluoroscopy imaging system.[Results] 1. All quality of images were met the diagnostic requirements:The quality of 93 cases between MRCP and ERCP had no significant difference(P> 0.05). 2. There were 17 cases of normal bile duct in morphology, and 76 cases of bile duct dilatation in all of observed patients. MRCP showed:the diameter of dilated bile duct was 12.64±2.60 mm(M±SD), MRCP showed:the diameter of dilated bile duct was 12.77±2.50 mm(M±SD), The diameter of 93 cases between MRCP and ERCP had no significant difference(P> 0.05).3. Comparison diagnostic accuracy of MRCP and ERCP in various diseases:The Chart 1 [Conclusion] 1. MRCP and ERCP techniques were both of important means in biliary and pancreatic system diseases, the accuracy of diagnosis between MRCP and ERCP was no difference.2. MRCP images were superior to ERCP images in cholec ystic diseases, especially for the display of cholecystic lithiasis better than ERCP. So can not rule out the possibility of the existence of cholecystic lithiasis when ERCP not show cholecyst and cholecystic lithiasis.3. MRCP was Still cannot replace ERCP. Especially in some pancreatic system diseases such as small Common bile duct and pyogenic cholangitis, for ERCP with EST technology Combined with the advantages of both diagnosis and treatment dual role and more advantages.4. MRCP can be used as the preferred examination in biliary and pancreatic system diseases, it should take further ERCP examination if the lesions could not be found on MRCP images to confirm the diagnosis. MRCP and ERCP combined with complementary, can improve diagnostic accuracy effectively.
