Objective By investigating the characteristics of the coronary angiography(CAG) finding coronary heart disease(CHD) complicated with type2 diabetes mellitus(DM) and non- diabetes mellitus(DM)(control groups),the study aims to demonstrate the relationship of coronary artery disease between diabetes mellitus(DM) and characteristics of injured coronary,and try to illuminate the value of diabetes mellitus(DM),which is a risk of coronary artery disease.We wish the study will provide datas and evidences for further prevention and remedy.Methods By retrospective case analysis,we study 1017 coronary artery disease cases admitted in cardiology department and coronary stensis≥50%diagnosed on cornary angiography from Jaunty.1.2007 to June.31.2007(except acute ST -segment elevation myocardial infaction cases)。Patients were divided into two groups with diabetes mellitus and without diabetes mellitus according to the last modified diabetes mellitus diagnostic criteria of ADA and WHO.We analyzed and compared two group in the genenral datas,clinical datas,serum markers and coronary angiographic findings.According to ACC / AHA coronary lesion morphology classification standards,Gensini score and SYNTAX score,we can qualitatively and quantitatively evaluated Coronary artery lensions。Results The mean ages of cases were 61.51±8.02(35-83 years).The DM guoup were 222 cases(21.83%) and non DM group were 795(78.17%).1.The proportion of female in DM group were higher than in non DM group(P=0.000).The proporttion of hypertension in DM group than in non DM group(P=0.000).2.The levels of FPG and FIB in DM group were highier than in non DM gourp(P=0.000; P=0.000).Conclusion there are more complicated and severe vessel lesions in CHD with type 2 diabetes mellitus(DM).3.Single vessel disease majored in non DM group,and multiple vessel disease majored in DM group.It is significantly diferent (P<0.01).4.The rate of severe stenosis was higher in DM group,It is significantly different(P<0.05).5.The incidence of diffuse change in DM group is significantly higher than thatof non DM group It is significantly different(P<0.001).6.Most of the vessel lesions are type B and type C,and the rate of type C was singificantly higher in DM group.It is significantly different(P<0.05).7.The proportion of compensatory circulation in DM group were higher than in non DM group.It is significantly different(P<0.05).8 the levels of the left ventricular end diastolic dimension in DM were higher than non DM,It is significantly different(P<0.05).the levels of LVEF in non DM group is higher than DM group.9.The levels of SYNTAX score and Ln of Gensini score in DM group were higher than non DM group,It is significantly different(P<0.05).10.The proportion of high SYNTAX score in PCI group were higher than in CABG group.It is significantly different(P<0.05).Conclusion1..The DM patients complicating CHD have more accompanied with risk factors such as hypertension,hyperglycemia and hyperfibrinogenemia.The proportion of female in DM group were higher than in non DM group.2.The DM patients complicating CHD have more accompanied with multiple vessel disease,diffuse change,complicated lesions and arteriostenosis.3.The DM patients complicating CHD have more accompanied with bypass circuit, because the drgree of their coronary artery stenosis were more serious.4.SYNTAX score can effective evaluate the serious of levels of coronary artery diseaseand provide assistance for the revascularization Strategy of coronary artery.
