A Clinical Analysis Of Non-Headache Symptoms In 71 Migraine Patients

Objects:Migraine is a severe disabling disorder. Headache is the most common symptom among migraine patients, in addition to headache, non-headache symptoms of migraine also affect the quality of life of patients suffering from migraine. In the present study, by using a self-made migraine questionnaire, we investigated types and prevalence of non-headache symptoms, and the correlation between non-headache symptoms and related factors during the migraine attack phase in 71 migraine patients, then proposed the possible pathogensis and provided the basis for the migraine mechanism.Methods:By using a self-made migraine questionnaire, we inquiried 71 patients in the age range of 16-60 years who primary treated in headache clinic, department of neurology, the second hospital of Jilin University between January 2010 to January 2011. All patients were diagnosed as having migraine(with or without aura) according to the criteria of International Classification of Headache Disorders,2nd edition,published by International Headache Society in 2004. The questionare contained sociodemographic factors, headache characteristics and non-headache symptoms,the sociodemographic factors included:age, sex and education;the characteristic of headache main contains degree,mean attack frequency per month, migraine subtype (with or without aura);the non-headache symptoms main contains 21 symptoms based on literature and clinical practice,we recorded the non-headache symptoms by using fixed inquiry model,then analysized the relevance among mean number of non-headache symptoms per individual and age,sex,education,migraine subtype(with or without aura),headache degree,mean attack frequency per month, analysized the prevalence of each non-headache symptoms in migraine patients and the relevance among the prevalence of each non-headache symptoms and sex, migraine subtype(with or without aura).The level of significance was set to p<0.05.Results:Of 71 migraine patients,non-headache symptoms were common. The mean number of non-headache symptoms per individual was 7.4±5.5,age,education, migraine subtype (with or without aura) and mean attack frequency per month had not statistically significant on the mean number of non-headache symptoms per individual (p>0.05).The most frequently reported non-headache symptoms were nausea (87.3%), phonophobia (81.7%) and mood change(69.0%).The prevalence of skin temperature and color changes,perivascular papules and defecation less than 10% and between 4.2%to 9.9%, the prevalence of other symptoms between 11.3% to 63.4%;The prevalence of phonophobia,sleep disorder, osmophobia in migraine without aura were higher than in migraine with aura (p<0.05), except these symptoms,age and migraine subtypes (with or without aura) had no statistically significant on other non-headache symptoms(p>0.05).Conclusions:Migraine attacks were often accompanied by varieties of non-headache symptoms;The effects of age,education, migraine subtypes(with or without aura) and mean attack frequency per month on mean number of non-headache symptoms per individual were not statistically significant;the most frequently reported non-headache symptoms were nausea,phonophobia and mood change;The prevalence of phonophobia,sleep disorder, osmophobia in migraine without aura were higher than in migraine with aura, except these symptoms,the effects of sex,migraine subtypes(with or without aura) on other non-headache symptoms were not statistically significant; the difference in inqury may influence the positive rate of non-headache symptoms.
