Objective:Cerebrovascular diseases have high morbidity, mortality, mutilation rate, it is constitute the three major causes of human death with heart disease, cancer together. Cerebral infarction Which is accounted for 80% of Cerebrovascular diseases ls a serious threat to human health. After treatment, there are about 50% to 70%patients with cerebral infarction leave paralysis, hemianopia, aphasia in varying degrees and other serious disabilities, that make society and the family carry a heavy burden. It is a strategic point that how to promote regeneration of nerve cells in the component of infarct, which promote Function of the nervous system and even complete recovery in patients with cerebral infarction. In recent years, stem cell transplantation for the treatment of central nervous system diseases created a new technology platform, there may be effective way to solve this world problems. This study from a clinical perspective, we discuss the efficacy and safety of stem cell transplantation for the treatment of sequelae of cerebral infarction through compared the conventional treatment group (control group) and stem cell transplantation in the treatment group (observation group).Methods:90 eligible patients were divided into two groups:the observation group accept stem cell transplantation, a total of 28 patients; 62 cases do not agree to accept the stem cell transplantation but only receive routine medication and rehabilitation training, that is the control group. Observation group admission NIHSS score was 9.96±3.697 points, BI index 36.43±9.512, control group admission NIHSS score was 9.42±3.371 points, BI index was 33.79±8.429, there were no statistically significant difference (P>0.05)between the two groups about their age, gender ratio, clinical condition and treatment time, cerebral infarction, and the degree of neurological deficit on Patient admission, these two groups have comparable. After the group accept stem cell transplantation signed informed consent procedure,we extracted sterile 160-200 ml of autologous bone under general anesthesia and by density gradient centrifugation, we extracted bone marrow mesenchymal stem cells from autologous bone. And then, we took the BMSCs after isolation and culture transfused back to the patient by means of lumbar puncture. Two groups of patients are to be gave NIHSS score and BI index score before operation and after operation 1 month,3 months,6 months. We set the data calculated with the mean x and standard deviation s, the data between groups using two sample t test. Measurement data to x±s in the form, using the t test.We Observed the patients with or without fever, headaches and other adverse reactions and took a long-term follow-up observation of patients with or without central nervous system tumors. Results:NIHSS score:the observation group and control group after stem cell transplantation 1 month,3 months,6 months,3 sets of data simultaneously between the two groups of two sample t test, p<0.05, it had significant statistically difference. After stem cell transplantation 1 month and 3 months and 3 months compared with 6 months, p<0.05, it had significant statistically difference.BI index score:the observation group and control group in stem cell transplantation after 1 month,3 months,6 months,3 sets of data between the two groups of two sample carried on simultaneously t test, p<0.05, it had significant statistically difference. After stem cell transplantation 1 month compared with its 3 months and its 3 months compared with its 6 months, p <0.05, it had significant statistically difference. Among 30 patients in the observation group, there were 4 cases sufferring from fever, their temperature<38°C, within 24 hours, they both returned to normal in body temperature. No other adverse reactions and complications arise. After 6 months follow-up observation, we were not found the incidence of central nervous system tumors. Conclusion:Based on 90 patients with a clinical control study of patients with cerebral infarction,we can draw the following conclusions:1)it can significantly improve neurological function in patients with cerebral symptoms through transplantation of autologous bone marrow mesenchymal stem cell.2)we can not discover obvious side effects by means of lumbar puncture transfuse autologous bone marrow mesenchymal stem cells.3)After the long-term follow-up observation, there is no central nervous system tumors occur, the method at preset safe and effective.
