| Cerebrovascular disease is common and frequently-occurring disease in the nervous system, with high incidence rate, high recurrence rate , high morbidity rate and high death rate features, which is the most serious harmful to human health disease currently. Ischemic cerebrovascular disease is the most common in cerebrovascular disease. 320-detector row CT turned out to be the gospel to the patients with ischemic cerebrovascular disease , "one- stop" scanning is of great value to the diagnosis and treatment of ischemic cerebrovascular disease.320-detector row CT developed with clinical need , with a wide range, short scanning time, differences in 0-phase characteristics. The CT imaging mode changes from "camera" to "high-definition camera," which is becoming dynamic volume scanning truly. In recent years, 320CT is widely used in the nervous system, "one-stop" scanning become the biggest bright spot with its unique advantages.The so-called "one-stop" check is that the patient can get a full set of clinical cerebral blood flow data including the head of the CT scanning images, CT angiography images and whole brain CT perfusion images through one CT scanning. CT perfusion images which can display the regional cerebral tissue perfusion defects corresponding to the damage nervous system early have great value of finding ICVD patients abnormal perfusion area and judging the scope of lesions; while CT angiography can display intracranial blood vessels clearly and judge the extent of vascular stenosis and the nature of plaque. Meanwhile, the joint application of CT angiography and CT perfusion imaging can combine the disease cause and the performance of ischemic cerebrovascular disease , and assess the extent of the damaged cerebral ischemia tissue effectively, which can enhance the sensitivity and specificity of diagnosis, and have a guiding significance to the treatment of ICVD.In this study, we chose the clinical diagnosis of TIA and cerebral infarction patients from November 2009 to February 2011 at the department of neurology and policlinic in China Japan Union Hospital of Jilin University as the study object . Select 20 TIA patients and 40 cerebral infarction patients all with clinical manifestations of carotid artery system symptoms into the TIA group and cerebral infarction group ,all the patients have to get the 320CT one-stop scanning,. Making a statistical analysis to the parameters obtained by CT perfusion imaging, all statistical data deal with SPSS17.0 package, x±s is applied for quantitative data, t test is applied in the test of significance of mean number. P <0.05 is seemed to as significantly different, while P <0.01 is seemed to as statistically significant difference.Study: In TIA group ,the parameters of rCBF , rCBV, MTT of lesion side compared with normal side p is greater than 0.05, with no significant difference; the parameters of TTP of lesion side compared with normal side p is less than 0.01, with a significant difference. In cerebral infarction group, the parameters of rCBF of irreversible ischemic group compared with reversible group p is greater than 0.05, with no significant difference ; while the parameters of rCBV of irreversible ischemic group compared with reversible group p is less than 0.01, with a significant difference. In cerebral infarction group, to the patients in theⅡandⅢstage of cerebral infarction radiology, the parameter ratios of rCBF, rCBV, MTT, TTP of lesion side compared with normal side of basal ganglia p is greater than 0.05, with no significant difference; to the patients in theⅡandⅢstage of cerebral infarction radiology, the parameter ratios of rCBF, rCBV, MTT, TTP of lesion side compared with normal side of cortex p is less than 0.05, with statistically significant.Conclusion:1. Of the 320CT perfusion imaging, TTP images can found the abnormal cerebral tissue perfusion more sensitive than CBF, CBV and MTT images of TIA patients, and have a great value of definiting the hypoperfusion region.2. 320CT one-stop scanning can judge the extent of ischemia of cerebral infarction patients region cerebral tissue according to the relationship of rCBF,rCBV , and have a significance of judging whether the cerebral tissue ischemia is reversible or not.3. The joint application of CTA and CTPI of 320CT one-stop scanning has a guiding significance to the treatment of cerebral infarction patients with stenosis . |
