Objective:With the rapid economic development,China and the world into an aging society。The extension of life expectancy and the high demands on quality of life is that people continue to pursue one of the requirements。The dementia quality of life for older persons has become apparent that people's concern for dementia diseases are growing。Dementia is an acquired, progressive cognitive dysfunction syndrome, and Alzheimer's disease (AD) is the most common dementia type。Dementia consciousness of the content rather than the level of awareness。The incidence of dementia and prevalence increase with age。Survey of foreign,Prevalence of dementia in people over the age of 60 to 1%,40% over the age of 85 or more。According to the report of prevalence of dementia in people over 60 years of age was 0.75%-4.69%。To achieve early detection,early diagnosis,early intervention is to improve the key to dementia diseases,It can also significantly reduce the hazards of AD, reduce the burden on society and the family。According to incomplete statistics,in 2006 worldwide was about 2600 million AD patients,in 2050, AD patients will be 4 times now, which means every 85 individualst, here is a AD patients, and to cause great social and family burden。The Diagnosis of AD is through neuropsychology, imaging biochemistry, molecular biology and clinical performance, the neuropsychological examination scale diagnosis and treatment of AD is an important indicator to measure effectiveness。Montreal Cognitive Assessment (MoCA) is a new scale for clinical cognitive impairment on screening and assessment scales。it has high reliability and validity,compared with the Mini-Mental State Examination(MMSE),it's more sensitive and specific,however, because its time of application in the domestic is short, so different parts has different results o This study examined the prevalence of AD and MoCA screening of dementia in the community, the feasibility of applying, also analysis the screening effect of MoCA for AD, discuss its reliability, validity, sensitivity and specificity。Discuss the best cutoff value,to assist in early detection of AD patients in the old people。Methods: Screen AD in community elderly in Chengdu by MMSE and MoCA, AD diagnostic criteria used by the U.S. National neurology, language barrier and Stroke-Alzheimer Disease and Related Disorders Association (NINCDS/ADRDA) criteria the diagnosis of possible AD。Calculate the detection rate of AD, analysis its influencing factors。Then calculate the reliability, validity, sensitivity, specificity, positive predictive value, negative predictive value and Youden's index of MoCA, discuss its screening value。Discuss the best cutoff value of the city scores for elderly patients with AD。Results: The total number of subjects was 674,the number of AD was44, and the detection rate of AD was 6.53%。This study shows that education level, age, sex, occupation of AD were statistically significant (P<0.01). The Cronbach's index was 0.852; the correlation coefficient between the MoCA and MMSE was 0.834; excepted place orientation and the connection, the score of other items had highly correlation to the total score (r>0.5)。Conclusion:The detection rate of AD is high in Chengdu community, aging and the low educational level are the risk factors for AD。The MoCA is a feasible screening tool for AD, According to the results of this study, Recommended by 15 points as the city community AD of the cut-off value。...
