Objective The study aims at investigating the effect of insulin intensive therapy on MHC expression in the liver and the levels of blood CD4+,CD8+ in diabetic critically ill rats.It provided theory basis for treating the diabetic critically ill patients.Methods 60 male Wistar rats were received intraperitoneal injection of 0.1% streptozotocin(60mg/kg) to establish diabetic models.30 diabetic rats were made into the trauma models. Rats with closed injury with fracture in both hind limbs were adopted as the trauma models.Then they were randomized into 2 groups:trauma intensive therapy group(n=15) and trauma conventional therapy group(n=15). Other 30 diabetic rats were made into the non-trauma models. Then they were randomized into 2 groups:non-trauma intensive therapy group(n=15) and non-trauma conventional therapy group(n=15).Blood glucose was maintained at the level of 3.9-5.6mmol/L in the intensive therapy group,and 10-12mmol/L in the conventional group.Blood was collected at pro-trauma,24h,7d,and the levels of CD4+,CD8+ were examined through flow cytometry.At the 7th day,the livers were collected,then the expression of MHC was observed by immunohistochemistry. Statistical treatment drawed assistance from SPSS12.0.Results 1. The levels of CD4+,CD4+/CD8+ in the group of intensification therapy are much higuer than that of conventional therapy while the levels of CD8+ have no obviously difference.2. The MHC expression in the liver are much higuer in the group of intensification therapy than that of conventional therapy,and have evident statistical significance.Conclusions Compared with the conventional therapy,insulin intensive therapy can significant elevate the expression of MHC in the livers and the levels of CD4+,CD4+/CD8+ in the blood through matained blood glucose at 3.9-5.6mmol/L. It can improve the immune of the organism and build a protective effect for the organism.
