Objective: This study based on traditional chinese medicine theory, for the spleen and kidney deficiency, phlegm and blood stasis is the key pathogenesis of refractory nephrotic syndrome,setting up a method which can invigorate spleen and kidney,remove blood and digest sputum.And planning yishengtang.Through observation of clinical cases to explore clinical effect of yishengtang on treating refractory nephrotic syndrome.Methods: The subjects were using the method of self-contrast before and after,50 patients with refractory nephrotic syndrome were compared before and after treatment. In the general treatment based on the increase taking yishentang. All the patients were treated for 8 weeks.The TCM syndromes were observed.Moreover, 24h urinary protein,serum albumin and other laboratory indices were measured and compared before and 8 weeks after starting the treatement.Results: (1)after treatment,the total effective rate was 80%, the effect of clinical symptoms of group after treatment were better than group before treatment (P<0.05).(2)The treatment could decrease the qualitative urine protein and 24-hour urine protein while raise serum albumin,the effect was better in group after treatment (P<0.01).(3)After treatment,cholesterol and triglyceride decreased,the difference was significant(P<0.05).(4)The treatment could prolong PT and APTT while lower FIB, the difference was significant (P<0.05).Conclusions:(1)after treatment,the total effective rate was 80%. During treatment, patients with no obvious adverse reaction.(2)yishentang can improve clinical symptoms markedly.(2)yishentang can help reduce urinary protein while increased serum albumin.(3)yishentang can effectively regulate dyslipidemia, improve blood hypercoagulability.
