ObjectiveThis study analyses 42 examples on conventional MRI and symptom after application DWI and DTI of Intervertebral Disc Degeneration in order to investigate the significance of injury status determinant of lumbar intervertebral disc degeneration and intervertebral disc annulus fibrosus based on application of magnetic resonance DWI and DTI and analysis parameter index. Intervertebral disc is a structure that is rich in water and no supply of blood, its nutritional component is provided primarily by proliferating of peripheral blood vessel. DWI(Diffusion Weighted Imaging)is a way of imaging activities of water diffusion in the human body, while ADC (apparent diffusion coefficients) is a parameter index of the extent of water diffusion. FA map is an examination that making sure the markedly changed status of diffusion anisotropy of the various parts of annulus fibrosis which is superior to conventional MRI. This study was the measurement of ADC values, determines the water spread extent in adult's lumbar disc nucleus pulposus, definitudes the correlationship between ADC values and MR which showed as intervertebral disc degeneration. There is a great significance of comprehensive evaluation of fiber bundles change by way of fiber tracking maps. Materials and MethodsDuring July 2009 to September 2009, this study selected 42 cases, ageing from 30 years to 65 years old with an average age of 45.5, which showed a medical history of low back pain, unilateral or bilateral sciatica and (+) result in straight leg raising test.5 healthy volunteers were male with an average age of 35 years old and had no above mentioned clinical symptoms. All patients were all using GE 1.5T superconductive MRI scanning system. Make conventional scanning of sagittal T1WI, T2WI. Horizontal axis is parallel to the disc T2WI. Sagittal scans the DWI. Make DTI of row axial. Using the Functool software in workstation to measure ADC value in ADC map. Place ROI in the lumbar sacral 1 to 5 discs in a bottom-up sequence and measure ADC values of each disc respectively. Correlation analysis is followed. Get FA (fractional anisotropy) value map by post-processing of DTI. Measure the FA values of continuous interruption in fibrous ring and fiber tracking map of normal group and patients with lumbar disc disease by setting annular circular ROI(regions of interest) both at the margin and lateral margin of front and rear discs respectively. Get fiber tracking map by reconstructing the software and setting annular circular ROI along the Intervertebral disc annulus fibrosus edges.ResultsThe ADC value is remarkablely reduce in the intervertebral disc degeneration and protrusion group, while in the normal group, some ADC value also have declined with the declining of intervertebral disc value due to the declining location. There is a correlationship between the average ADC value of normal intervertebral disc and the location of intervertebral disc. FA value in normal and degenerate group in intervertebral disc fibrosus posterior edge is lower than those in front and both sides with no obvious features, but the FA value significantly declines in the part of intervertebral disc protrusion (especially in pulposus prolapse). DTI technology can clearly show the shape and integrity of intervertebral disc anulus fibrosus. It showed a continuous complete ring structure in normal disc fiber tracking map. From the fiber tracking maps of the pathological process in intervertebral disc back edge, we can see that fiber lose continuous status.ConclusionADC value and FA value, as a quantitative analysis method combined the application of diffusion tensor fiber tractography imaging technology, can be successfully applied to the display of intervertebral disc anulus fibrosus. It has an important clinical value of preoperative evaluation, treatment options and evaluation of efficacy after treatment.
