Clinical Theraphy Analysis Of Pancreatic Duct Stone (12 Cases Reports)

Objective To study the feature of clinic and surgrical treatment in pancreatic duct stones.Methods The clinica1 data of 12 patients treated form 2002～2010 were retrospectively analyzed. the whole group patients generally check for the abdomen ultrasound ,and doubt PDS or the specific cases to determine the specific situation check the MRCP, check the pancreas tumor with the increase CT, determine to remove the stones with endoscopes were checked with ERCP . Summarize the treatment of pancreatic duct stone (PDS) and operation time. Operations were carefully designed and carried according to complications and stone location. Therapy method:( i) medical management As a common treatment, it generally include temperance,low-fat diet,medicinal(insulin,creon) ec al ;(ii) Endoscopic retrograde cholangiopancrtography (ERCP) using techniques was performed to manage residual stones, and stone diameter< 0.8cm , no pancreatic duct stricture;(iii)Operation styles include Patington,Du val,Whipple ec al.Results According to the imaging examination and operative findings, PDS were classified into 4 different types with respective manners of surgical treatment. 10 of 12 cases underwent operation , ERCP was used to remove the stones in 1case,3cases who complicated with pancreatic tumor underwent pancreatolithotomy ,3cases underwent plus side to side Roux2en2Y pancreaticojejunostomy,distal pancreatectomy with splenectomy in one case ,1case underwent lateral pancreaticoje- junostomy asscociate with cholangio-jejunostomy.12cases abdominal pain or discomfort obtained the relief, operated 9 cases were obvious,and no pancreatic fluid leakage happened.
