Aims: The early diagnosis of acute aortic syndromes(AAS) remains challenging. We sought to determine the utility of the serum D-dimer level, as a diagnostic biomarker of acute aortic syndromes.Methods: We retrospectively collected 78 cases who were finally diagnosed of AAS between January 2009 and February 2011 , including 54 patients with acute aortic dissection(AAD),23 patients with intramural hematoma(IMH) and 1 patient with penetrating atherosclerotic ulcer(PAU). The control group included 40 patients with pulmonary embolism(PE) and 40 patients with acute myocardial infarction(AMI). We analyzed the D-dimer level in these patients.The method of D-dimer test is latex agglutination test, and the kit is produced by sekisuimedical corporation. The reference threshold is 0.232ug/ml and the cut-off in this study is 0.5ug/ml,which is widely accepted by most scientists. We use SPSS13 for windows and EXCEL to analysis data.Results: The P50 D-dimer levels of AAS(AAD+IMH) group, AAD group, IMH group, control(PE+AMI) group, AMI group and PE group were 2.4ug/ml, 3.51ug/ml,1.96ug/ml,1.65ug/ml,1.15ug/ml,2.83ug/ml,respectively. Analysis the differences of D-dimer in AAS group versus all control groups showed P value were 0.018,<0.001 and 0.96 versus control group, AMI group and PE group, respectively. Analysis the differences of D-dimer in AAD group versus IMH group and all control groups showed P value were 0.028,0.003,<0.001 and 0.433 versus IMH group, control group, AMI group and PE group, respectively. Analysis the differences of D-dimer in IMH group versus all control groups showed P value were 0.846,0.055 and 0.116 versus control group, AMI group and PE group, respectively.Conclusions: D-dimer level is markedly elevated in AAS, which indicates D-dimer may be useful in early diagnosis of AAS.D-dimer level of IMH is significant lower than that of AAD, but is similar to that of AMI. We can't differentiate acute myocardial infarction and intramural hematoma by D-dimer in clinical work.
